摘要
目的研究中西医结合卒中单元治疗缺血性脑卒中恢复期的临床疗效。方法选取2016年7月—2018年12月就诊于南昌市洪都中医院内科的100例缺血性脑卒中恢复期患者,随机分为治疗组和对照组,每组50例。2组在基础治疗上,治疗组予以热敏灸加"醒脑开窍"针刺结合卒中单元治疗,对照组予以卒中单元治疗。比较2组的临床治疗效果、简易精神状态检查(Mini-mental state examination,MMSE)评分、脑卒中病损评估记录(Stroke impairment assessment set,SIAS)评分、Barthel指数(Barthel index,BI)评分及牛津残障量表(Oxford handicap score,OHS)等级。结果治疗组的治疗总有效率高于对照组,差异有统计学意义(P <0.05);治疗组第15、30日的MMSE、SIAS、BI评分高于对照组,差异均有统计学意义(P <0.05);治疗组的OHS等级优于对照组,差异有统计学意义(P <0.05)。结论热敏灸加"醒脑开窍"针刺结合卒中单元治疗缺血性脑卒中恢复患者疗效明显,能有提高治疗效果,改善患者神经功能缺损及伤残程度,值得临床推广运用。
Objective To study the clinical effect of integrated traditional Chinese and western medicine stroke unit on the convalescence ischemic stroke. Methods A total of 100 convalescent patients with ischemic stroke treated in Nanchang Hongdu Hospital of Traditional Chinese Medicine from July 2016 to December 2018 were selected and randomly divided into treatment group and control group,with 50 patients in each group. On the foundation treatment in both groups, the patients in the control group were given stroke unit and patients in the treatment group were given moxibustion at heat sensitive and Xingnao Kaiqiao acupuncture combined with stroke unit.Clinical effect, MINI-Mental State Examination(MMSE) score and stroke impairment assessment, SIAS score, Barthel Index(BI)score and Oxford Handicap Score(OHS) grade were compared between the two groups. Results The total effective rate in treatment group was higher than that in control group, and the difference was statistically significant(P <0.05). MMSE, SIAS and BI scores in the treatment group were higher than those in the control group on the 15 th and 30 th day, the differences were statistically significant(P <0.05). OHS grade of the treatment group was better than that of the control group, and the difference was statistically significant(P <0.05). Conclusion Heat sensitive moxibustion and Xingnao Kaiqiao acupuncture combined with stroke unit has obvious curative effect on patients recovering from ischemic stroke, which can improve the therapeutic effect and improve the neurological function defect and disability degree of patients, and is worthy of clinical application.
