摘要
目的探讨关节镜下改进双Endobutton钢板治疗RockwoodⅢ〜Ⅴ肩锁关节脱位的疗效。方法回顾性分析2016年8月至2019年1月24例RockwoodⅢ〜Ⅴ型肩锁关节脱位患者临床资料,所有患者采用关节镜下改进双Endobutton钢板行喙锁韧带重建术。观察关节功能恢复情况,采用Bjemld肩锁关节脱位评价系统进行术后肩锁关节功能评价:结果24例患者术后获得随访,时间8〜24个月,平均(11.6±2.4)个月。所有患者肩部外形均正常,无肩锁关节复位丢失。锁骨及喙突无医源性骨折:根据Bjemeld肩锁关节脱位评价系统:优20例,良4例结论RockwoodⅢ〜Ⅴ型肩锁关节脱位患者,关节镜下改进双Endobutton钢板内固定重建喙锁軔带,具有微创,医源性骨折风险小,局部稳定性佳,肩锁关节功能恢复满意。
Objective To investigate the clinical effect of improved double Endobutton plate fixation in the treatment of Rockwood HI V acromioclavicular joint dislocation by arthroscope.Methods the clinical data of 24 cases of Rockwood typeⅢ〜Ⅴacromioclavicular j oint dislocation treated in our hospital from August 2016 to January 2019 were selected to investigate the mid and long-term clinical efficacy of improved double Endobutton plate fixation by arthroscope.There were 18 males and 6 females,aged from 21 to 55 years[(35.7+2.0)years].There were 13 cases on the left:side and 11 cases on the right side.Rockwood classification:14 cases were typeⅢand 10 cases were typeⅤ.The average time&om injury to operation was(3.4+1.2)days(range,2~7 days).Arthroscopic reconstruction,of coracoclavicular ligament with improved double Endobutton plate was performed.The clinical symptoms were observed,and the acromioclavicular joint dislocation score system was used to evaluate the shoulder joint function.Results All patients were followed up for 8 to 24 months(mean 11.6+2.4 months).All patients had no infection and rejection,the incision healed in the first stage.At the last follow-up,all patients'shoulder pain disappeared in daily activities,the tenderness of acromioclavicular joint was not obvious,and the piano key sign was negative.The X-ray showed that the reduction of acromioclavicular joint dislocation was maintained well,no subluxation occurred,and no loosening or displacement ofEndobutton plate occurred.There was no iatrogenic fracture of clavicle and coracoid process.According to the Bjemeld acromioclavicular dislocation evaluation system,20 cases were excellent and 4 cases were good.Conclusion The patients with RockwoodⅢ〜Ⅴacromioclavicular joint dislocation,arthroscopic reconstruction of coracoclavicular ligament with improved double Endobutton loopless plate fixation is an ideal treatment method with minimally invasive operation,low risk of clavicle and coracoid process fracture,few complications and good functional recovery.
