摘要
目的对比分析Endobutton带袢钛板与锁骨钩钢板在治疗RockwoodⅢ型及以上肩锁关节脱位的临床疗效。方法回顾性分析我院收治的RockwoodⅢ型及以上肩锁关节脱位患者43例,按照固定方式不同将其分为两组。观察组22例采用Endobutton带袢钛板弹性固定,对照组21例采用锁骨钩钢板刚性固定。比较两组患者手术指标、Constant-Murley肩关节功能评分、喙锁间隙、肩锁间隙、肩关节活动度及术后并发症等情况。结果两组手术时长及住院时间比较,P>0.05,观察组切口长度、术中失血量均小于对照组(P<0.05)。两组术后3、6、12个月肩关节功能评分较术前均逐渐增高(P<0.05),其中观察组在术后3、6、12个月各时间点的Constant-Murley肩关节功能评分均高于对照组(P<0.05)。与术前对比,术后3、6、12个月,两组喙锁间隙(CC)与肩锁间隙(AC)均显著减小(P<0.05),术后12个月时,两组CC、AC比较,P<0.05;术后3、6个月,两组间CC及AC比较,P>0.05。随时间推移,两组患者的肩关节活动度均显著提升(P<0.05);且观察组患者肩关节活动度均显著高于对照组(P<0.05)。观察组术后并发症发生率显著低于对照组(P<0.05)。结论对RockwoodⅢ型及以上肩锁关节脱位患者,Endobutton带袢钛板弹性固定较钩钢板刚性固定手术切口小、失血量及术后并发症少,且无需再次手术取出内置物,有利于肩关节功能恢复。
Objective To compare the therapeutic effect of Endobutton loop plate and clavicular hook plate in the treatment of Rockwood type III and above acromioclavicular joint dislocation.Methods Clinical data of 43 patients with Rockwood type III and above acromioclavicular joint dislocation admitted to our hospital were retrospectively analyzed.They were divided into two groups according the fixation methods.22 cases in the observation group were fixed with Endobutton loop plate,21 cases in the control group were fixed with clavicular hook plate.The surgical indicators,Constant-Murley shoulder joint function score,coracoclavicular space,acromioclavicular space,motion range of the shoulder joint and postoperative complications were compared between the two groups.Results There were no statistically significant difference in operation time and hospitalization time between the two groups(P>0.05).The incision length and intraoperative blood loss in the observation group were smaller than those of the control group(P<0.05).3,6 and 12 months after operation,the Constant-Murley shoulder joint function scores of the two groups all increased gradually compared with those before operation(P<0.05),the score in the observation group were higher than those in the control group(P<0.05).The coracoclavicular space(CC)and acromioclavicular space(AC)significantly reduced in both groups at 3,6 and 12 months after operation compared with those before surgery(P<0.05).12 months after surgery,there were statistically significant differences in CC and AC between the two groups(P<0.05),but 3 and 6 months after surgery there were no statistically significant differences between the two groups(P>0.05).Over time,the motion range of the shoulder joint in both groups increased significantly(P<0.05);Moreover,the motion range of the shoulder joint in the observation group was significantly higher than that in the control group(P<0.05).The incidence of postoperative complications in the observation group was significantly lower than those in the control group(P<0.05).Conclusion Endobutton loop plate fixation is better than clavicular hook plate fixation for patients with Rockwood type III and above acromioclavicular joint dislocation,which has smaller incision,less intraoperative blood loss,better shoulder joint function and less complications.
