摘要
目的通过强化营养支持护理,加强对行内镜下硬化剂治疗食管胃底静脉曲张破裂出血患者的治疗,探讨治疗效果和预后情况。方法选取2018年1月至2020年1月因肝硬化食管胃底静脉曲张出血行内镜下硬化剂治疗术的患者60例,采用随机分组法分为对照组和观察组,对照组患者给予流食7 d+普通饮食2周的营养支持,观察组在对照组的基础上,给予复方氨基酸3周和1周加肠内营养制剂序贯2周睡前加餐进行营养干预,4周后比较2组患者的治疗效果和并发症发生情况。结果观察组患者SGA 1级和2级的发生率(80%)明显高于对照组(40%),观察组治疗4周后BMI指数较对照组高,差异有统计学意义(P<0.05)。观察组患者创面愈合率(93.3%)较对照组高(60%),而并发症如注射点出血、溃疡、穿孔及发热明显低于对照组,差异有统计学意义(P<0.05)。结论强化营养支持护理可改善内镜下硬化剂治疗食管胃底静脉曲张破裂出血患者的营养状况,增强治疗效果,降低并发症发生率,提高患者生活质量,可在临床上广泛推广。
Objective To investigate the application effects of intensive nutrition support nursing in endoscopic sclerotherapy for esophageal and gastric varices bleeding.Methods A total of 60 patients with esophageal and gastric varices bleeding due to cirrhosis who were treated by endoscopic sclerotherapy in our hospital from January 2018 to January 2020 were enrolled in the study,who were randomly divided into control group and observation group.The patients in control group were treated by nutritional support of 7-day liquid diet+2-week general diet,however,the patients in observation group,on the basis of control group,were treated by 3-week compound amino acid+1-week sequential enteral nutrition+2-week extra meal before bedtime for nutrition intervention.After 4-week treatment the therapeutic effects and the incidence of complications were observed and compared between the two groups.Results The incidence rate of SGA1 and SGA2 in the observation group was 80%,which was significantly higher than that(40%)in control group,moreover,the BMI in observation group was significantly higher than that control group(P<0.05).The wound healing rate(93.3%)in observation group was significantly higher than that(60%)in control group,and the incidence rates of complications including bleeding at the injection site,ulcer,perforation and fever in observation group were significantly lower than those in control group(P<0.05).Conclusion The intensive nutrition support nursing can improve the nutritional status of patients with esophageal and gastric varices bleeding due to cirrhosis who are treated by endoscopic sclerotherapy,which can improve the therapeutic effects,reduce the incidence of complications,improve the life quality of patients,therefor,which is worth using widely in clinical practice.
