摘要
目的探究联合检测血清淀粉样蛋白A(SAA)、SAA与血清淋巴细胞计数比值(SAA/L)、D-二聚体水平对新型冠状病毒肺炎预后的判断价值。方法收集2020年1月18日至2020年2月26日该院通过病原学核酸检测阳性的116例新型冠状病毒肺炎确诊患者资料,研究其入院时、住院3~5 d、复合终点时外周血SAA、SAA/L、D-二聚体的动态变化,与转归进行对比分析。结果死亡患者SAA、SAA/L、D-二聚体3个时间点检测水平均明显高于出院患者。复合终点时SAA+SAA/L+D-二聚体三者联合区分出院和死亡患者的曲线下面积(AUC)最高,为0.995(95%CI:0.985~0.998),而SAA/L为0.981(95%CI:0.956~0.987)、SAA为0.969(95%CI:0.934~0.986),D-二聚体为0.928(95%CI:0.823~0.948),且死亡患者3个指标水平均持续增高,尤其D-二聚体在死亡前达最高峰,出院患者则持续下降。结论SAA+SAA/L+D-二聚体三者联合检测对新型冠状病毒肺炎预后的判断价值高。
Objective To explore the prognostic value of combined detection of serum amyloid A(SAA),ratio of SAA to serum lymphocyte count(SAA/L),D-dimer levels in patients with corona virus disease 2019(COVID-19).Methods The data of 116 patients who were admitted to hospital from January 18,2020 to February 26,2020 were collected.Studied the dynamic changes of peripheral blood SAA,SAA/L,and D-dimer at the time of admission,3—5 days of hospitalization,and composite end point,compared and analyzed the results.Results The levels of SAA,SAA/L and D-dimer at these three time points in dead patients were significantly higher than those in discharged patients.At the composite endpoint,area under the curve(AUC)of SAA+SAA/L+D-dimer in distinguishing discharge and death was the highest with 0.995(95%CI:0.985—0.998),while SAA/L was 0.981(95%CI:0.956—0.987),SAA/L was 0.981(95%CI:0.956—0.987),SAA is 0.969(95%CI:0.934—0.986),D-dimer was 0.928(95%CI:0.823—0.948),and the three indicators of dying patients continued to increase,especially D-dimer reached the highest peak before death,while discharged patients continued to decline.Conclusion The combined detection of SAA+SAA/L+D-dimer is of high value in judging the prognosis of COVID-19.
