摘要
目的采用定量磁敏感图(QSM)方法检查高血压患者脑内铁沉积情况,探究影响其异常脑铁沉积的因素。方法92例高血压患者及40名正常志愿者接受磁共振检查,获得QSM图像和一般资料,勾画脑内灰质核团和额叶白质的感兴趣区(ROI),并测量其磁敏感值。比较两组受试者之间及左右半球之间各脑区磁敏感值的差异。采用多重线性回归研究高血压患者各项临床因素与各脑区磁敏感值的关系。结果高血压组在尾状核、壳核、齿状核的磁敏感值高于对照组,在丘脑的磁敏感值低于对照组(P<0.05)。受试者双侧脑区磁敏感值均未见明显差异(P>0.05)。高血压患病年限、既往最高收缩压、体质指数(BMI)、性别、年龄、糖尿病、吸烟对高血压患者不同脑区磁敏感值有显著性的影响(P<0.05)。结论高血压患者部分脑区铁含量有升高改变,多个因素会显著影响其脑内铁沉积,应通过改变相关生活方式来改善脑铁异常沉积。
Objective Quantitative susceptibility mapping(QSM)was used to examine the iron deposition in the brain of hypertensive patients and the purpose of this research was to explore the factors affecting the abnormal iron deposition in the brain.Methods 92 hypertensive patients and 40 normal controls were recruited for MRI examination,by which QSM images and clinical data were obtained.The regions of interest(ROI)of gray matter nuclei and frontal white matter were delineated and their susceptibility were measured.The differences of susceptibility between the two groups and between the bilateral hemispheres were compared.Multiple linear regression was used to analyze the relationship between the clinical factors and the susceptibility of each brain region in hypertensive patients.Results The susceptibility of caudate nucleus,putamen and dentate nucleus in hypertension group were higher than those in control group,but lower than those in control group in thalamus(P<0.05).However,there was no significant difference in the susceptibility between the bilateral hemispheres(P>0.05).In contrast,years of hypertension,previous highest systolic blood pressure,BMI,gender,age,history of diabetes and smoking history had significant effects on the susceptibility of each brain region in hypertension patients(P<0.05).Conclusion The iron content of some brain region in patients with hypertension was increased,and many factors can significantly affect the iron deposition in the brain,which can be improved by changing the related lifestyles.
