摘要
目的探讨血清抗苗勒管激素(AMH)、血清脂肪特异性丝氨酸蛋白酶抑制剂(Vaspin)、鸢尾素(Irisin)水平对多囊卵巢综合征(PCOS)的评估价值。方法选取2017年1月至2018年12月信阳市第一人民医院接收的140例疑似PCOS的患者作为研究对象,所有患者均通过《多囊卵巢综合征中国诊疗指南》判定其PCOS发生情况,测定患者实验室指标[血清甘油三酯(TG)、总胆固醇(TC)、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、促黄体生成激素(LH)、促卵泡生成素(FSH)、AMH、Vaspin、Irisin],重点分析血清AMH、Vaspin、Irisin在PCOS患者中的检测价值。结果140例疑似PCOS的患者中有60例确诊为PCOS,占比42.86%。与非PCOS组相比,PCOS组血清AMH、Vaspin水平明显升高,血清Irisin水平明显降低(P<0.05);组间年龄,BMI、孕产史以及激素水平相关基线资料相比,差异无统计学意义(P>0.05)。绘制ROC曲线,结果显示,血清AMH、Vaspin、Irisin诊断PCOS的AUC>0.7,具有一定的诊断价值,且联合诊断的效能最好。结论血清AMH、Vaspin、Irisin在评估PCOS中有一定的价值,且以联合诊断的效能最好。
Objective To explore the value of serum anti-Müllerian hormone(AMH),serum visceral adipose specific serine protease inhibitor(Vaspin)and Irisin levels in the evaluation of polycystic ovary syndrome(PCOS).Methods A total of 140 patients with suspected PCOS in the First People’s Hospital of Xinyang City from January 2017 to December 2018 were selected as the research subjects,and all patients were judged the occurrence of PCOS according to the Chinese Guidelines for Diagnosis and Treatment of Polycystic Ovary Syndrome.The laboratory indexes[serum triglyceride(TG),total cholesterol(TC),fasting plasma glucose(FBG),hemoglobin A1c(HbA1c),luteinizing hormone(LH),follicle stimulating hormone(FSH),AMH,Vaspin and Irisin]of patients were measured.The detection value of serum AMH,Vaspin and Irisin in patients with PCOS were mainly analyzed.Results Among 140 cases of patients with suspected PCOS,and 60 cases were diagnosed as PCOS,accounting for 42.86%.Compared with non PCOS group,the levels of AMH and Vaspin in PCOS group were significantly increased,while the level of Irisin was significantly decreased(P<0.05).There were no statistical significant differences in age,BMI,history of pregnancy and childbirth and hormone level between the two groups(P>0.05).The ROC curve was drawn,and the results showed that the AUC of serum AMH,Vaspin and Irisin in the diagnosis of PCOS were>0.7,which had certain diagnostic value,and the combined diagnosis was the best.Conclusion Serum AMH,Vaspin and Irisin are valuable in the evaluation of PCOS,and the efficiency of combined diagnosis is the best.
