自制拇指伸展矫形器联合点阵激光治疗烧伤患者手部虎口瘢痕挛缩的效果观察 被引量:4

Self-Made Thumb Extension Orthosis Combined with Dot-Matrix Laser Treatment for Scar Contracture of Hand between Thumb and Index Finger in Burn Patients

摘要 目的:观察自制拇指伸展矫形器联合点阵激光治疗烧伤患者手部虎口瘢痕增生后挛缩的疗效。方法:选择昆山市康复医院2018年1月~2019年12月收治的虎口部烧伤瘢痕增生后挛缩患者32例,以随机数字表法分为联合组和点阵激光组各16例,均接受烧伤后的常规治疗,其中点阵激光组采用点阵激光治疗,治疗频率为1次/2月;联合组则在每次激光治疗后配合自制拇指伸展矫形器进行干预治疗,每天佩戴不低于12h,6个月后采用温哥华瘢痕评估量表、拇指伸展角度差值和患者满意度视觉模拟评估量表得分评估两组患者手部虎口瘢痕增生后挛缩疗效。结果:治疗前,两组患者温哥华瘢痕评估量表得分差异无统计学意义(P>0.05);治疗后,两组患者温哥华瘢痕评估量表评分均较治疗前显著改善(P<0.05),且联合组得分显著低于点阵激光组(P<0.05);联合组拇指伸展角度差值和患者满意度视觉模拟评估量表得分显著高于点阵激光组(P<0.05)。结论:烧伤患者手部虎口增生后瘢痕经过自制拇指伸展矫形器联合点阵激光治疗后,虎口瘢痕挛缩和瘢痕质量明显改善,疗效满意。 Objective:To observe the effect of self-made thumb extension orthosis combined with dot-matrix laser treatment on scar contracture of the hand between the thumb and the index finger in burn patients.Methods:From January 2018 to December 2019,32 patients with scar contracture of the hand between the thumb and the index finger were selected from the Kunshan Rehabilitation Hospital,who were divided into combined group(n=16) and dot-matrix laser group(n=16) by random number table.Patients in both groups received routine treatment after burn,the dot-matrix laser group received dot-matrix laser treatment for scar contracture with a frequency of 1 time/2 months,while the combined group treated with self-made thumb extension orthosis for intervention treatment after every laser treatment.The efficacy of scar contracture was evaluated before and after treatment,including the Vancouver Scar assessment scale,thumb extension angle and the patient satisfaction visual analogue scale.Results:Before treatment,there was no significant difference between the two groups(P>0.05).After treatment,the scores of the two groups’ VSS were significantly improved(P<0.05),and also the scores of combined group were significantly lower than those of dot-matrix laser group(P<0.05);the thumb extension angle and the satisfaction’s VAS of combined group were significantly higher than those of dot-matrix laser group(P<0.05).Conclusion:The scar contracture and scar quality could be improved obviously after the treatment of self-made thumb extension ortho sis and dot-matrix laser.
