摘要
目的探究消痞和胃方结合四联疗法对幽门螺杆菌(HP)相关性慢性萎缩性胃炎患者疗效、胃功能、血清炎性因子水平影响。方法选取2019年6月—2020年2月医院收治的82例HP相关性慢性萎缩性胃炎患者纳入研究,采用随机数字表法分为对照组41例和观察组41例,对照组患者采取四联疗法治疗,观察组患者采取四联疗法结合消痞和胃方治疗,比较两组患者临床治疗效果、HP清除率、治疗前后患者中医证候(上腹部疼痛、胃胀、疲倦乏力、消瘦、舌红少津、口干口渴、大便干燥、脉细等)评分变化、治疗前后两组患者胃黏膜评分变化及胃蛋白酶原(PGⅠ及PGⅡ)含量变化、血清胃肠激素水平变化、血清炎症因子水平变化,采取健康调查简表(SF-36)中生理健康(PCS)及心理健康(MCS)内容评估患者治疗前后身心健康情况,观察治疗不良反应。结果观察组患者治愈率(92.68%,38/41)高于对照组(73.17%,30/41)(P<0.05);观察组患者HP清除率(100.00%,41/41)高于对照组(90.24%,37/41)(P<0.05);治疗前,两组患者中医证候总积分、胃黏膜评分、胃蛋白酶原(PGⅠ)含量、血清胃肠激素水平、血清炎症因子水平、PCS及MCS评分等数据比较,P>0.05,治疗后各组患者中医证候总积分、胃黏膜评分、胃蛋白酶原(PGⅠ)含量、血清胃肠激素水平、血清炎症因子水平、PCS及MCS评分等指标均改善,观察组患者治疗后中医证候总积分、胃黏膜评分、胃蛋白酶原(PGⅠ)含量、血清胃肠激素水平、血清炎症因子水平、PCS及MCS评分等指标优于对照组(P<0.05);治疗过程中,两组患者均未见不良反应出现。结论消痞和胃方结合四联疗法治疗HP相关性慢性萎缩性胃炎疗效佳,患者症状改善,胃功能恢复良好,生活质量改善,未见不良反应,安全性高,值得推广与应用。
Objective To explore the effects of Xiaopi Hewei Formula(消痞和胃方) combined with quadruple therapy on the efficacy, gastric function and serum inflammatory factors of Helicobacter pylori(HP)-related chronic atrophic gastritis. Methods In the study, 82 patients with HP-related chronic atrophic gastritis in our hospital from June 2019 to February 2020, according to the average random number table method, were incorporated into the control group and observation group. The control group(41 cases) was treated with quadruple therapy. The observation group(41 cases) was treated with quadruple therapy combined with Xiaopi Hewei Formula. Two groups’ clinical effect, HP clearance, TCM symptom(upper abdominal pain, bloating, fatigue, lack of power, angular, red tongue and lack of fluid, dry mouth, thirst, dry stool, veins thin, etc.) score changes, gastric mucosa score and contents of pepsinogen(PGⅠ and PGⅡ), serum gastrointestinal hormone level changes and changes in serum levels of inflammatory cytokines were compared. The physical health(PCS) and mental health(MCS) of health survey profiles(SF-36) were used for evaluating physical and mental health. The treatment of adverse reactions were observed. Results The cure rate of the observation group(92.68%,38/41) was higher than that of the control group(73.17%,30/41)(P<0.05). The HP clearance rate in the observation group(100.00%,41/41) was higher than that in the control group(90.24%,37/41)(P<0.05). Before treatment, two groups’ TCM syndrome integral, gastric mucosa score, PGⅠ content, serum gastrointestinal hormone levels, serum levels of inflammatory factors, PCS and MCS scores had no significant(P>0.05). After treatment, traditional Chinese medicine syndrome integral, gastric mucosa score, PGⅠcontent, gastrointestinal hormone levels, serum levels of proinflammatory factor, PCS and MCS scores were improved. After treatment, the observation group’s TCM syndrome integral, gastric mucosa score, PGⅠcontent, gastrointestinal hormone levels, serum levels of proinflammatory factor, PCS and MCS scores were better than those of the control group(P<0.05). During the treatment, no adverse reactions were found in both groups. Conclusion The combination of Xiaopi Hewei Formula and quadruple therapy in the treatment of HP-related chronic atrophic gastritis has a good curative effect. The patients’ symptoms are improved. The gastric function is restored well, the quality of life is improved. No adverse reactions are found and the safety is high. It is worthy of promoting and applying.
