血清CYFRA21-1、NSE、CEA、CA125及SCCA联合检测在肺癌诊断中的价值研究 被引量:6

Study on the Value of Combined Detection of Serum CYFRA21-1,NSE,CEA,CA125 and SCCA in the Diagnosis of Lung Cancer

摘要 目的分析诊断肺癌时检测血清细胞角蛋白19片段(CYFRA21-1)、神经元特异性烯醇化酶(NSE)、癌胚抗原(CEA)、糖类抗原125(CA125)和鳞状上皮细胞癌抗原(SCCA)水平作用。方法方便选择2018年2月—2020年2月在该院实施肺癌治疗的65例患者为研究样本,将其作为研究组。另选择同时期在该院实施健康体检的73名健康体检者为常规1组以及同时期在该院实施肺部良性疾病治疗的62例患者为常规2组。使用电化学发光法对每组血清SCCA、CA125、CEA、NSE以及CYFRA21-1水平实施评估,比较3组上述血清肿瘤标志物水平差异以及肺癌不同类型上述指标差异,分析单项肿瘤标志物检测同5项联合检测在诊断肺癌方面的准确度、灵敏度以及特异性。结果研究组血清SCCA(13.52±3.96)μg/L、CA125(70.75±20.31)U/mL、CEA(10.72±3.02)ng/mL、NSE(36.28±5.92)ng/mL以及CYFRA21-1(23.81±7.08)ng/mL水平均高于常规1组(1.87±0.43)μg/L、(8.46±1.62)U/mL、(2.09±0.61)ng/mL、(10.06±2.95)ng/mL、(2.20±0.63)ng/mL和常规2组(2.21±0.70)μg/L、(9.75±2.13)U/mL、(2.45±0.71)ng/mL、(11.02±3.21)ng/mL、(2.71±0.70)ng/mL,差异有统计学意义(P<0.05)。鳞癌在血清SCCA以及CYFRA21-1方面水平(20.82±6.15)μg/L、(31.87±10.23)ng/mL均高于腺癌(12.29±2.66)μg/L、(22.06±6.85)ng/mL、小细胞癌(12.52±3.06)μg/L、(23.05±7.13)ng/mL,差异有统计学意义(P<0.05);小细胞癌在血清NSE方面水平(44.85±10.23)ng/mL均高于腺癌(32.19±6.08)ng/mL、鳞癌(33.82±5.71)ng/mL,差异有统计学意义(P<0.05)。5型指标联合检测准确度(85.00%)以及灵敏度(83.78%)均高于单一检测方式。结论肺癌患者血清SCCA、CA125、CEA、NSE以及CYFRA21-1水平均有明显升高,联合检测上述指标可提升诊断效能,为临床治疗提供参考。 Objective To analyze the detection of serum cytokeratin 19 fragment(CYFRA21-1),neuron-specific enolase(NSE),carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125)and squamous epithelium when diagnosing lung cancer Cell cancer antigen(SCCA)level effect.Methods Conveniently selected 65 patients undergoing lung cancer treatment in the hospital were selected as the research samples,and they were used as the study group.The research time was between February 2018 and February 2020.In addition,73 health examiners who performed physical examinations in this hospital during the same period were selected as the conventional group 1 and 62 patients who received treatment for benign lung diseases in this hospital during the same period were selected as the conventional group 2.The electrochemiluminescence method was used to evaluate the serum levels of SCCA,CA125,CEA,NSE and CYFRA21-1 in each group.The differences in the levels of the above-mentioned serum tumor markers in the three groups and the differences in the above-mentioned indicators of different types of lung cancer were compared,and was the accuracy,sensitivity and specificity of the detection of individual tumor markers was and the combined items test in the diagnosis of lung cancer.Results The study group’s serum SCCA(13.52±3.96)μg/L,CA125(70.75±20.31)U/mL,CEA(10.72±3.02)ng/mL,NSE(36.28±5.92)ng/mL and CYFRA21-1(23.81±7.08)ng/mL was higher than the conventional group 1(1.87±0.43)μg/L,(8.46±1.62)U/mL,(2.09±0.61)ng/mL,(10.06±2.95)ng/mL,(2.20±0.63)ng/mL and conventional group 2(2.21±0.70)μg/L,(9.75±2.13)U/mL,(2.45±0.71)ng/mL,(11.02±3.21)ng/mL,(2.71±0.70)ng/mL,the difference was statistically significant(P<0.05).Squamous cell carcinoma had higher levels of serum SCCA and CYFRA21-1(20.82±6.15)μg/L,(31.87±10.23)ng/mL than adenocarcinoma(12.29±2.66)μg/L,(22.06±6.85)ng/mL,small cell carcinoma(12.52±3.06)μg/L,(23.05±7.13)ng/mL,the difference was statistically significant(P<0.05);serum NSE levels of small cell carcinoma(44.85±10.23)ng/mL were higher than adenocarcinoma(32.19±6.08)ng/mL,squamous cell carcinoma(33.82±5.71)ng/mL,the difference was statistically significant(P<0.05).The accuracy(85.00%)and sensitivity(83.78%)of the combined detection of type 5 indicators were higher than that of the single detection method.Conclusion The levels of serum SCCA,CA125,CEA,NSE and CYFRA21-1 in lung cancer patients are significantly increased.Combined detection of the above indicators can improve the diagnostic efficiency and provide references for clinical treatment.
