摘要
目的:探究血液透析滤过(HDF)、血液透析(HD)+血液灌流(HP)治疗尿毒症顽固性高血压的效果。方法:选取2018年1月-2020年6月笔者所在医院收治的86例尿毒症顽固性高血压患者。根据随机数字表法将其分为A、B组,每组43例。A组实施HDF治疗,B组实施HD+HP治疗。比较两组治疗效果。结果:治疗前,两组血压水平比较差异无统计学意义(P>0.05);治疗5周时,B组血压水平低于A组,差异有统计学意义(P<0.05)。治疗前,两组全段甲状旁腺激素、血钙、血磷水平比较差异均无统计学意义(P>0.05);治疗5周时,B组全段甲状旁腺激素、血磷水平均低于A组,血钙水平高于A组,差异有统计学意义(P<0.05)。A组不良反应发生率为9.30%,B组不良反应发生率为4.65%,组间比较差异无统计学意义(P>0.05)。结论:应用HD+HP治疗尿毒症顽固性高血压,可改善患者血压水平及血液质量,且安全性理想,具有临床应用价值。
Objective:To explore the effect of hemodiafiltration(HDF),hemodialysis(HD)+hemoperfusion(HP)in the treatment of uremic refractory hypertension.Method:A total of 86 patients with uremia refractory hypertension admitted in our hospital from January 2018 to June 2020 were selected.According to random number table method,they were divided into group A and group B,with 43 cases in each group.Group A was treated with HDF,while group B was treated with HD+HP.The therapeutic effect of the two groups was compared.Result:Before treatment,there was no significant difference in blood pressure between the two groups(P>0.05).After 5 weeks of treatment,the blood pressure of group B was lower than that of group A,and the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of parathyroid hormone,serum calcium and serum phosphorus between the two groups(P>0.05).After 5 weeks of treatment,the levels of parathyroid hormone and serum phosphorus in group B were lower than those in group A,and the level of serum calcium in group B was higher than that in group A,the differences were statistically significant(P<0.05).The incidence of adverse reactions in group A was 9.30%,and 4.65%in group B,and there was no significant difference between the two groups(P>0.05).Conclusion:The application of HD+HP in the treatment of uremia refractory hypertension can improve the patient’s blood pressure level and blood quality,and the safety is ideal,and has clinical application value.
