摘要
目的探讨不同方式治疗晚期血吸虫合并食管胃底静脉曲张的临床效果。方法选取300例晚期血吸虫合并食管胃底静脉曲张为研究对象,并随机分为药物组,非精准治疗组和精准治疗组,每组各100例。药物组给予传统药物治疗,非精准治疗组给予行三明治法(聚桂醇-组织胶-高糖)血管旁黏膜下注射,精准治疗组给予行三明治法(聚桂醇-组织胶-高糖)血管内注射,比较两组总有效率、止血成功率,早期再出血率、迟发性再出血率及出血复发率。结果三组总有效率及止血成功率比较结果为精准治疗组>非精准治疗组>药物组,早期再出血率、迟发性再出血率及出血复发率比较结果为药物组>非精准治疗组>精准治疗组,差异均具有统计学意义(P<0.05)。结论晚期血吸虫合并食管胃底静脉曲张破裂出血急诊行内镜下精准断流临床效果显著,建议形成常规治疗手段减少死亡率。
Objective To explore the clinical effects of different treatments for advanced schistosomiasis combined with esophageal and gastric varices.Methods 300 cases of advanced schistosomiasis combined with esophagogastric varices were selected as the research object,and randomly divided into drug group,non-precision treatment group and precision treatment group,100 cases in each group.The drug group was given traditional drug treatment,the non-precision treatment group was given a sandwich method(polycinnamic alcohol-tissue glue-high sugar)subvascular injection,and the precision treatment group was given a sandwich method(polycinnamic alcohol-tissue glue-high sugar).Intravascular injection,compare the total effective rate,hemostatic success rate,early rebleeding rate,delayed rebleeding rate and bleeding recurrence rate of the two groups.Results The comparison results of total effective rate and success rate of hemostasis among the three groups were precision treatment group>non-precision treatment group>drug group,and the results of early rebleeding rate,delayed rebleeding rate and bleeding recurrence rate were drug group>non-precision treatment group>The precision treatment group,the differences were statistically significant(P<0.05).Conclusion The clinical effect of endoscopy with accurate endoscopic debridement for advanced schistosomiasis with esophagogastric varices rupture is urgent.It is recommended to use conventional treatment to reduce mortality.
