摘要
目的研究代谢综合征患者胰岛素抵抗与循环中GPC4水平变化的关系。方法选取2019年6—12月在重庆市铜梁区人民医院肾脏内分泌科门诊及病房代谢综合征患者56例,将其设定为代谢综合征组,依据体质量指数(BMI)将代谢综合征人群分为3个亚组,即正常体重组(BMI﹤25 kg/m^(2),n=20)、超重组(BMI≥25 kg/m^(2),n=20),肥胖组(BMI≥30 kg/m^(2),n=16),同期健康体检者52名作为对照组,统计分析各组人员的体质量、BMI、腰围、血压、血脂、FPG、FINS、HOMA-IR、GPC-4水平及其相关性。结果代谢综合征组患者的体质量、BMI、腰围、SBP、DBP均显著高于对照组,差异有统计学意义(P<0.05),正常体重组、超重组、肥胖组患者的体质量、BMI、腰围、SBP、DBP逐渐升高,差异有统计学意义(P<0.05)。代谢综合征组患者的TG水平显著高于对照组,差异有统计学意义(P<0.05),HDL-C水平显著低于对照组,差异有统计学意义(P<0.05);正常体重组、超重组、肥胖组患者的TG、TC水平均逐渐升高,差异有统计学意义(P<0.05)。代谢综合征组中正常体重组、超重组、肥胖组、对照组人员的HOMA-IR水平分别为(3.7±1.2)、(4.4±1.3)、(5.5±1.6)、(1.7±0.5),GPC-4水平分别为(1.7±0.5)、(2.1±0.6)、(2.4±0.6)、(1.4±0.4)ng/mL。代谢综合征组患者的FPG、FINS、HOMA-IR、GPC-4水平均显著高于对照组,差异有统计学意义(P<0.05),正常体重组、超重组、肥胖组患者的FPG、FINS、HOMA-IR、GPC-4水平逐渐升高,差异有统计学意义(P<0.05)。循环中GPC-4水平与体质量、BMI、腰围、TG、FINS、HOMA-IR均呈显著的正相关关系(P<0.05),与HDL-C呈显著的负相关关系(P<0.05);HOMA-IR与体质量、BMI、腰围、SBP、DBP、TG、TC、LDL-C、FPG、FINS、GPC-4均呈显著的正相关关系(P<0.05),与HDL-C呈显著的负相关关系(P<0.05)。结论代谢综合征患者胰岛素抵抗与循环中GPC4水平呈显著的正相关关系。
Objective To study the relationship between insulin resistance and changes in circulating GPC4 levels in patients with metabolic syndrome.Methods From June to December 2019,56 patients with metabolic syndrome in the outpatient and ward of the renal endocrinology department of People's Hospital of Tongliang District,Chongqing City were selected and set as the metabolic syndrome group.The metabolic syndrome population was divided into three subgroups based on BMI.That was,normal body reorganization(BMI﹤25 kg/m^(2),n=20),overweight reorganization(BMI≥25 kg/m^(2),n=20),obesity group(BMI≥30 kg/m^(2),n=16),52 healthy subjects in the same period served as the control group,statistical analysis of the body weight,BMI,waist circumference,blood pressure,blood lipid,FPG,FINS,HOMA-IR,GPC-4 levels and their correlation in each group.Results The body weight,BMI,waist circumference,SBP and DBP of patients in the metabolic syndrome group were significantly higher than those of the control group,the difference was statistically significant(P<0.05).The body weight,BMI,waist circumference,SBP and DBP of patients in the normal,overweight,and obese groups gradually increased,the difference was statistically significant(P<0.05).The TG level of patients in the metabolic syndrome group was significantly higher than that of the control group,the difference was statistically significant(P<0.05),and the level of HDL-C was significantly lower than that of the control group,the difference was statistically significant(P<0.05);the TG and TC levels of the patients in the normal body,overweight,and obese groups were significantly increased,the difference was statistically significant(P<0.05).In the metabolic syndrome group,the HOMA-IR levels of the normal recombination,overweight recombination,obesity group,and control group were(3.7±1.2),(4.4±1.3),(5.5±1.6),(1.7±0.5),GPC-4 levels were(1.7±0.5)ng/mL,(2.1±0.6)ng/mL,(2.4±0.6)ng/mL,(1.4±0.4)ng/mL.The levels of FPG,FINS,HOMA-IR,and GPC-4 in the metabolic syndrome group were significantly higher than those in the control group,the difference was statistically significant(P<0.05),and the FPG,FINS,HOMA-IR,GPC-4 level in the normal body,overweight and obese groups gradually increased,the difference was statistically significant(P<0.05).GPC-4 levels in circulation were significantly positively correlated with body weight,BMI,waist circumference,TG,FINS,and HOMA-IR(P<0.05),and significantly negatively correlated with HDL-C(P<0.05);HOMA-IR has a significant positive correlation with body weight,BMI,waist circumference,SBP,DBP,TG,TC,LDL-C,FPG,FINS,GPC-4(P<0.05),and a significant negative correlation with HDL-C(P<0.05).Conclusion There is a significant positive correlation between insulin resistance and circulating GPC4 levels in patients with metabolic syndrome.
