摘要
目的:研究初诊2型糖尿病患者及肥胖者血清腹腔脂肪型丝氨酸蛋白酶抑制剂(vaspin)及视黄醇结合蛋白-4(RBP-4)水平变化及与肥胖、2型糖尿病代谢变化之间的关系及其临床意义。方法:选取我院初诊2型糖尿病患者(T2DM)60例,按照BMI水平分为正常体重糖尿病组(T2DM-NW组)及超重肥胖糖尿病组(T2DM-OB组),另选健康体检者60例作为对照组(NGT),同样分为正常体重组(NGT-NW组)及超重肥胖组(NGT-OB组),采用ELISA检测血清vaspin及RBP-4水平。结果:各组间血清vaspin、RBP-4水平依次为:T2DM-OB组>NGT-OB组>T2DM-NW组>NGT-NW组,差异均有统计学意义(P<0.05)。Pearson相关性分析提示vaspin与体重、BMI、腰围、WHR、FIns、胰岛素抵抗指数(HOMA-IR)呈显著相关性(r分别为0.533,0.595,0.603,0.622,0.530,0.536,P<0.01);RBP-4与体重、BMI、腰围、BMI、FIns、HOMA-IR呈显著相关性(r分别为0.533,0.595,0.603,0.622,0.530,0.536,P<0.01);血清vaspin与RBP-4呈显著正相关(P<0.01)。多元线性逐步回归分析显示BMI、WHR和FIns是vaspin独立相关因素;BMI、WHR和HOMA-IR是血清RBP-4的独立相关因素。结论:血清vaspin和RBP-4与HOMA-IR、BMI、WHR呈显著正相关,提示该两种脂肪源性细胞因子可能在T2DM、胰岛素抵抗和肥胖的发生发展中起一定的作用。
Objective:To study the changes of serum visceral adipose tissue-derived serine protease inhibitor( vaspin) and retinol binding protein-4( RBP-4) in newly diagnosed type 2 diabetic patients; and to explore the relationship between vaspin,RBP-4 and the metabolic parameter changes of type 2 diabetes mellitus and obesity and its clinical significance.Methods:60 T2DM patients( T2DM group) were randomly divided into T2DM-NW group and T2DM-OB group according to BMI level.60 healthy subjects( NGT group) were chosen as NGT-NW group and NGT-OB group.Serum vaspin and RBP-4 were detected by ELISA.Results:The levels of serum vaspin and RBP-4 in each group were:T2DM-OB group > NGT-OB group > T2DM-NW group > NGT-NW group,and the difference was statistically significant( P < 0.05).The correlation analysis showed that vaspin was significantly correlated with body weight,BMI,waist circumference,WHR,FIns and HOMA-IR( r = 0.533,0.595,0.603,0.622,0.530,0.536,P < 0.01).RBP-4 was significantly correlated with body weight,BMI,waist circumference,WHR,FIns and HOMA-IR( r = 0.533,0.595,0.603,0.622,0.530,0.536,P < 0.01).Serum Vaspin was positively correlated with RBP-4( P < 0.01).The results of multiple linear stepwise regression showed that BMI,WHR and FIns were independent influencing factors of serum vaspin; and BMI,WHR and HOMA-IR were independent influencing factors of serum RBP-4.Conclusion:Serum vaspin and RBP-4 are positively correlated with HOMA-IR,BMI and WHR,suggesting that these two adipocytokines may play a role in the development of T2DM,insulin resistance and obesity.
