摘要
目的调查某岛礁医院住院收治情况,分析疾病分布特点,为合理配置医疗资源、促进疾病防治、有效提高卫勤保障水平提供参考。方法提取某岛礁医院2020年2—10月住院收治患者资料,结合同期门急诊患者情况,参照ICD-10国际疾病分类标准对数据进行系统分类。结果共纳入59例次患者,占同期门急诊就诊患者比例0.74%(59/8013)。系统疾病谱分析前3位疾病分别是损伤、中毒和外因的某些其他后果42.4%(25/59)、泌尿生殖系统疾病20.3%(12/59)、消化系统疾病16.9%(10/59),合计占62.7%(47/59)。单病种分析疾病构成分布居前3位的分别是损伤(12)、骨折(6)、急性阑尾炎(6)。住院患者收治专科分布情况居前3位的分别是骨科25.4%(15/59)、泌尿外科20.3%(12/59)、肛肠外科13.6%(8/59)。住院患者手术率为27.1%(16/59),排名前3位的手术为阑尾切除术37.5%(6/16)、清创缝合术31.3%(5/16)、皮瓣转移术12.5%(2/16)。住院患者后送率为8.5%(5/59)。结论结合岛礁医院住院患者收治特点及转归情况,卫勤保障中手术相关的专科医生需配备,同时建议补足战创伤诊疗的相关硬件设备,可减少后送对患者治疗时机的延误以及巨大经济消耗,提高岛礁医院常规医疗服务质量和战创伤急救效能。
Objective To investigate the hospitalization situation of patients in an island hospital and ana⁃lyze the specialty composition and characteristics of disease distribution,so as to provide reference for rational allo⁃cation of medical resources,further promotion of disease prevention and control,and effective improvement of mili⁃tary medical support level.Methods The relevant data of inpatients in an island hospital from February to October 2020 were extracted and classified according to International Classification of Diseases Tenth Edition(ICD-10).Results A total of 59 patients with complete hospitalization records were included,accounting for 0.74%(59/8013)of the outpatient and emergency patients during the same period.The top three disease spectrums were inju⁃ry,poisoning and other consequences of external causes(42.4%or 25/59),urogenital system diseases(20.3%or 12/59),and digestive system diseases(16.9%or 10/59),with all the three accounting for 62.7%in total.The top three single diseases were injury(12),fracture(6)and acute appendicitis(6).The top three inpatient de⁃partments were orthopedics department(25.4%or 15/59),urology department(20.3%or 12/59),colorectal sur⁃gery department(13.6%or 8/59).The operation rate was 27.1%(16/59).The top three operations were appen⁃dectomy(37.5%or 6/16),debridement and suture(31.3%or 5/16),and flap transfer(12.5%or 2/16).The referral rate was 8.5%(5/59).Conclusion Based on the characteristics of hospitalized patients and their followup outcomes in the island hospital,it is suggested that the operation-related specialized surgeons in the military medical support work should be dispatched routinely in the future and relevant hardware equipment for war trauma assessment and treatment be fully equipped,so as to reduce the referral delay and huge economic loss caused and to improve the quality of routine medical services and the efficacy of trauma medical first aid in the island hospital.
