摘要
目的分析肺炎性假瘤、周围型肺癌CT征象特征及其鉴别诊断价值。方法收集我院2018年1月至2019年4月收治的75例肺部疾病患者,其中肺炎性假瘤41例,周围型肺癌34例。所有患者均进行CT检查。对患者所得CT图像进行分析,观察两组患者影像学征象主要有:病灶位置(右肺、左肺)、病灶大小、CT征象(空泡征、支气管征、空洞、钙化、分叶征、毛刺征、平直征、血管集束征、胸膜凹陷征)情况等。结果肺炎性假瘤、周围型肺癌在病灶分叶征、毛刺征、平直征、血管集束征、胸膜凹陷征上比较差异存在统计学意义(P<0.001),肺炎性假瘤患者平直征为80.48%,多于周围型肺癌患者;周围肺癌患者分叶征91.17%,毛刺征82.35%,血管集束征、胸膜凹陷征均为79.41%,明显高于肺炎性假瘤患者。结论分叶征、毛刺征、平直征、血管集束征、胸膜凹陷征为肺炎性假瘤、周围型肺癌鉴别重要征象,CT检查对两者诊断鉴别价值高。
Objective To analyze the CT features of pulmonary inflammatory pseudotumor and peripheral lung cancer and its diagnostic value.Methods 75 patients with lung diseases treated in our hospital from January 2018 to April 2019 were collected,including 41 cases with pulmonary inflammatory pseudotumor and 34 cases with peripheral lung cancer.All patients underwent CT.The CT images of the patients were analyzed.The imaging signs of the two groups were mainly:lesion location(right lung,left lung),lesion size,CT signs(vocule sign,bronchus sign,cavity,calcification,lobulation sign,burr signs,flat sign,vascular bundle sign,pleural depression sign)and so on.Results There were statistically significant differences in the lobular sign,burr signs,flat sign,vascular bundle sign,pleural depression sign between pulmonary inflammatory pseudotumor and peripheral lung cancer(P<0.001).The proportion of patients with pulmonary inflammatory pseudotumor who had straight signs was 80.48%,which was higher than that of patients with peripheral lung cancer.The proportion of patients with peripheral lung cancerwho had lobular sign was 91.17%,had burr sign was 82.35%,and had vascular bundle sign and a pleural depression sign was 79.41%,which were higher than those of patients with pulmonary inflammato ry pseudotumor.Condusion The lobula r sign,burr sign,straight sign,vascular bundle sign,and pleural depression sign are importa nt signs for identifying of pulmona ry inflammato ry pseudotumor from peripheral lu ng cancer.CT exa mination is of high diagnostic value for them.
