摘要
目的探讨床旁超声可视化教学在内科住院医师规范化培训中的应用效果。方法以24名规培医师为研究对象,分为可视组和对照组,每组各12人。以股静脉穿刺为教学内容,可视组采用床旁超声定位可视化股静脉穿刺术教学,对照组采用解剖定位盲探股静脉穿刺术教学,教学结束后,对两组规培医师进行每人5例操作考核,分别记录两组股静脉穿刺总成功率、一次穿刺成功率和误伤动脉率。并对每位规培医师进行主观问卷调查,问卷内容包括解剖知识掌握、操作自信心和教学满意度评价。结果可视组总成功率为93.33%,高于对照组的56.67%,差异有统计学意义(P<0.001);可视组一次穿刺成功率为71.67%,高于对照组的35.00%,差异有统计学意义(P<0.001);可视组误伤动脉率为3.33%,低于对照组的21.67%,差异有统计学意义(P<0.01)。问卷调查结果显示,与对照组相比,可视组规培医师对解剖知识掌握更好、操作自信心更强、教学满意度更高,差异有统计学意义(P<0.001)。结论床旁超声可视化教学为内科住院医师规范化培训提供了安全有效的教学手段和方法,具有良好的教学效果。
Objective To explore the application effect of bedside ultrasound visualization teaching in standardized training of residents in internal medicine.Methods A total of 24 physicians of standardized training were selected as the study subjects and divided into the visual group and the control group,with 12 subjects in each group.The femoral vein puncture was taken as the teaching contents,the visual group was given bedside ultrasound positioning to visualize the teaching of femoral vein puncture.The control group was given the teaching of anatomical positioning to blindly probe the femoral vein puncture.After teaching,5 sets of operation assessments for each physician in the two groups were conducted.The total success rate of femoral vein puncture,the success rate of one-time puncture and the rate of accidentally injured arteries were recorded in both groups respectively,and subjective questionnaire survey was given to each physician.The content of the questionnaire included:the degree of knowledge of anatomy,the degree of confidence in operation and the evaluation of teaching satisfaction.Results The total success rate in the visual group was 93.33%,the control group was 56.67%.The total success rate in the visual group was significantly higher than that in the control group(P<0.001);the success rate of one-time puncture in the visual group was 71.67%,and the control group was 35.00%.The success rate of one-time puncture in the visual group was significantly higher than that in the control group(P<0.001);the rate of accidentally injured arteries in the visual group was 3.33%,and the control group was 21.67%.The rate of accidentally injured arteries in the visual group was significantly lower than that in the control group(P<0.01).The results of questionnaire survey showed that compared with the control group,the physicians in the visual group had a better grasp of anatomy knowledge,stronger self-confidence in operation,and higher satisfaction with teaching(P<0.001).Conclusion The bedside ultrasound visualization teaching provided safe and effective teaching approaches and methods for the standardized training of internal medicine residents,and had good teaching effects.
