摘要
目的探讨老年心力衰竭患者血浆galectin-3表达与心脏结构、功能的关系。方法选取2018年7月至2020年1月新疆医科大学第一附属医院收治的老年心力衰竭患者102例,美国纽约心脏病学会(NYHA)分级为Ⅰ级19例、Ⅱ级26例、Ⅱ-Ⅳ级57例。心脏彩色多普勒超声测量心脏结构和功能参数,按左心室射血分数(LVEF)分为LVEF≤40%组(n=35)、LVEF>40%组(n=67),按左心室舒张末期内径(LVEDD)分为LVEDD<50 mm组(n=19)、LVEDD 50~70 mm组(n=60)、LVEDD>70 mm组(n=23),分别进行血浆galectin-3和N末端脑钠肽原(NT-proBNP)水平组间比较。心功能NYHA III-IV级患者使用常规抗心力衰竭治疗,按疗效分为无效组和有效组,比较两组血浆galectin-3和NT-proBNP水平。比较不同NYHA分级患者血浆galectin-3、NT-proBNP水平和超声心动图参数。结果LVEF≤40%组血浆galectin-3、NT-proBNP水平均显著高于LVEF>40%组(均P<0.05)。血浆galectin-3水平随LVEDD增加而升高(均P<0.05)。LVEDD 50~70 mm组血浆NT-proBNP水平高于LVEDD<50 mm组(P<0.05)。心功能NYHAⅢ-Ⅳ级患者治疗后疗效评价显示有效51例、无效6例。有效组治疗后血浆NT-proBNP、galectin-3水平低于治疗前,无效组治疗后血浆NT-proBNP、galectin-3水平高于治疗前(均P<0.05)。心功能NYHA分级Ⅱ级、III-IV级组患者血浆galectin-3、NT-proBNP及LVEDD水平均高于Ⅰ级组,左心室缩短分数(LVFS)、LVEF低于Ⅰ级组(均P<0.05)。患者血浆galectin-3、NT-proBNP水平随心功能分级增加而升高(均P<0.05)。结论老年心力衰竭患者血浆galectin-3表达与心脏结构、功能改变有关联。
Objective To investigate the correlation between plasma galectin-3 expression and cardiac structure and function in elderly patients with heart failure.Methods A total of 102 elderly patients with heart failure admitted to the First Affiliated Hospital of Xinjiang Medical University between July 2018 and January 2020 were included in the study.According to the New York Heart Association(NYHA)functional classification,all patients were divided into classⅠ(n=19),Ⅱ(n=26)andⅢ-Ⅳ(n=57).Cardiac color Doppler ultrasonography was used to measure the cardiac structure and function parameters.According to the left ventricular ejection fraction(LVEF),the patients were divided into the LVEF≤40%group(n=35)and the LVEF>40%group(n=67).According to the left ventricular end-diastolic diameter(LVEDD),the patients were divided into LVEDD<50 mm group(n=19),LVEDD 50-70 mm group(n=60),and LVEDD>70 mm group(n=23).The plasma galectin-3 and N-terminal pro-B-type natriuretic peptide(NT-proBNP)levels were compared between groups,respectively.The patients with NYHA classⅢ-Ⅳreceived conventional anti-heart failure treatment,and were divided into invalid group and effective group according to the efficacy.The plasma galectin-3 and NT-proBNP levels in the two groups were compared.The plasma galectin-3 and NT-proBNP levels and echocardiographic parameters in patients with different NYHA classes were compared.Results The plasma galectin-3 and NT-proBNP levels in the LVEF≤40%group were significantly higher than those in the LVEF>40%group(both P<0.05).The plasma galectin-3 level increased along with the increasing LVEDD(P<0.05).The plasma NT-proBNP level in the LVEDD 50-70 mm group was higher than that in the LVEDD<50 mm group(P<0.05).The evaluation for efficacy in patients with NYHA classⅢ-Ⅳafter the treatment showed that there were 51 effective cases and 6 ineffective cases.After the treatment,the plasma NT-proBNP and galectin-3 levels in the effective group were lower than those at baseline,whereas the levels in the ineffective group were higher than those at baseline(all P<0.05).The plasma galectin-3 and NT-proBNP levels and LVEDD in NYHA classⅡandⅢ-Ⅳgroups were higher than those in the classⅠgroup,whereas the left ventricular fractional shortening(LVFS)and LVEF were lower than those in classⅠgroup(all P<0.05).The plasma galectin-3 and NT-proBNP levels increased along with the increasing cardiac function classes(all P<0.05).Conclusion The expression of plasma galectin-3 in elderly patients with heart failure is correlated with the changes in cardiac structure and function.
