摘要
目的:探讨半乳糖凝集素3在心肌梗死后慢性心力衰竭(心衰)患者血清中的表达水平及其与患者心功能的相关性。方法:选取2016-01至2017-06我院收治的108例心肌梗死后慢性心衰患者(观察组)。另选取同期我院收治的110例慢性心衰但无心肌梗死病史患者(对照组),均为高血压性慢性心衰。应用酶联免疫吸附试验法检测两组患者血清中半乳糖凝集素3的表达水平,采用超声心动图检测两组患者心功能相关指标,相关性分析采用Pearson分析法。结果:与对照组比较,观察组患者血清半乳糖凝集素3水平及左心室舒张末期内径(LVEDD)均较高,左心室射血分数(LVEF)较低,差异均有统计学意义(P均<0.05)。观察组中,有47例患者为LVEF下降性慢性心衰(LVEF≤40%者),有61例患者为LVEF保留性慢性心衰(LVEF>40%者),两者半乳糖凝集素3、左心室短轴缩短率(LVFS)、LVEDD及左心房内径(LAD)比较,差异均有统计学意义(P均<0.05)。Pearson相关性分析结果提示,观察组患者血清中半乳糖凝集素3水平与LVEF、LVFS呈负相关(P<0.05),与LVEDD、LAD呈正相关(P<0.05),而对照组患者血清中半乳糖凝集素3水平与其心功能指标无明显相关性(P>0.05)。结论:半乳糖凝集素3在心肌梗死后慢性心衰患者血清中呈高表达,其与患者心功能存在相关性,对临床评估该类患者的病情具有重要作用。
Objectives: To investigate the expression of serum galectin-3 in chronic heart failure(CHF) patients after myocardial infarction(MI) and its correlation to cardiac function.Methods: Our research included in 2 groups: CHF group, 108 CHF patients after MI admitted in our hospital form 2016-01 to 2017-06 and Control group, 110 hypertensive CHF patients without MI admitted in our hospital at the same period. Serum levels of galectin-3 were examined by ELISA, relevant cardiac function parameters were measured by echocardiography; correlation study was conducted by Pearson analysis.Results: Compared with Control group, CHF group had increased serum level of galectin-3 and left ventricular end diastolic diameter(LVEDD), decreased left ventricular ejection fraction(LVEF), all P〈0.05. CHF group had 2 subgroups: LVEF reduced subgroup, 47 patients with LVEF ≤ 40% and LVEF preserved subgroup, 61 patients with LVEF40%; serum level of galectin-3, left ventricular short axis fractional shortening(LVFS), LVEDD and left atrial diameter(LAD) were different between 2 subgroups, all P〈0.05. Pearson correlation analysis indicated that in CHF group, galectin-3 was negatively related to LVEF, LVFS and positively related to LVEDD, LAD, all P〈0.05; while in control group, galectin-3 had no obvious correlation to those cardiac function parameters, P〈0.05.Conclusions: Expression of serum galectin-3 was increased in CHF patients after MI; it was related to cardiac function and having an important effect for evaluating the clinical condition in relevant patients.
