摘要
目的总结术前栓塞联合显微手术治疗高级别脑动静脉畸形(BAVM)的效果。方法选取2016年1月至2018年12月我院收治的58例高级别BAVM患者为研究对象,按随机数字表法将其分为对照组和治疗组,每组29例。对照组单纯行显微手术治疗,治疗组在Onxy胶栓塞后再行显微手术治疗。比较两组患者的病灶全切率、复发率、术后神经功能障碍发生率及术中出血量。结果治疗组与对照组的全切率分别为93.10%、89.66%,两组全切率比较,差异无统计学意义(P>0.05);治疗组与对照组的复发率分别为3.70%、3.85%,两组复发率比较,差异无统计学意义(P>0.05)。治疗组术后神经功能障碍发生率明显低于对照组,术中出血量少于对照组,差异具有统计学意义(P<0.05)。结论Onxy胶栓塞与显微手术联合治疗高级别BAVM患者的效果较好,可减少术中出血量,降低术后神经功能障碍的发生率,值得临床应用与推广。
Objective To summarize the effect of preoperative embolization combined with microsurgery in the treatment of high-grade brain arteriovenous malformation(BAVM).Methods A total of 58 patients with high-grade BAVM treated in our hospital from January 2016 to December 2018 were selected as the research objects,and the patients were divided into control group and treatment group according to the random number table method,with 29 cases in each group.The control group was treated with microsurgery alone,and the treatment group was treated with microsurgery after embolization with Onyx glue.The total resection rate of lesion,recurrence rate,incidence of postoperative neurological dysfunction and the amount of intraoperative blood loss of the two groups were compared.Results The total resection rates of the treatment group and the control group were 93.10%and 89.66%respectively,with no significant difference between the two groups(P>0.05);the recurrence rates of the treatment group and the control group were 3.70%and 3.85%respectively,with no significant difference between the two groups(P>0.05).The incidence of postoperative neurological dysfunction in the treatment group was significantly lower than that in the control group,and the amount of intraoperative blood loss was less than that in the control group,the differences were statistically significant(P<0.05).Conclusion The Onyx glue embolization combined with microsurgery in the treatment of high-grade BAVM patients has effect better,it can reduce the amount of intraoperative blood loss and the incidence of postoperative neurological dysfunction,which is worthy of clinical application and promotion.
