摘要
目的:探讨逍遥蒌贝散对乳腺癌根治术患者外周血T淋巴细胞及引流液炎症指标的影响。方法:选择128例乳腺癌根治术患者,随机分为对照组和观察组,每组64例,对照组术后给予CMF方案(环磷酰胺、甲氨蝶呤、5-氟尿嘧啶)化疗,观察组基于以上加逍遥蒌贝散治疗,观察两组治疗4周后的临床疗效,对比治疗前及治疗4周后的中医症候积分、外周血T淋巴细胞亚群水平、引流液炎症指标含量。结果:观察组的客观缓解率(ORR)为68.78%,显著高于对照组的50.00%(P<0.05);治疗4周后,两组脘胁胀痛、食少、抑郁烦躁及太息共4项中医症候积分均显著低于治疗前(均P<0.05),观察组的外周血CD4+T淋巴细胞亚群占比及CD4+/CD8+T淋巴细胞亚群比值均显著高于治疗前(均P<0.05),两组引流液中白细胞介素(IL-8)、肿瘤坏死因子-α(TNF-α)含量均显著低于治疗前(均P<0.05),观察组变化幅度显著大于对照组(P<0.05)。结论:予逍遥蒌贝散治疗乳腺癌根治术后患者能提高临床疗效,缓解其相关中医症状,有效调节T淋巴细胞,减轻其引流液炎症反应。
Objective:To investigate the effects of Xiaoyao Loubei powder on peripheral T lymphocytes and drainage fluid inflammatory indicators in patients undergoing radical mastectomy.Methods:The data of 128 patients undergoing radical mastectomy for breast cancer were selected,and the patients were randomly divided into control group and observation group,with 64 cases in each group.Control group was given CMF chemotherapy regimen(cyclophosphamide,methotrexate,fluorouracil)after surgery,and observation group was given Xiaoyao Loubei powder on this basis.The clinical efficacy after 4 weeks of treatment was observed in the two groups,and the scores of TCM syndromes,levels of T lymphocyte subsets in peripheral blood and contents of inflammatory indicators in drainage fluid were compared before treatment and after 4 weeks of treatment.Results:The objective response rate(ORR)in observation group was significantly higher than that in control group(68.78%vs 50.00%,P<0.05).After 4 weeks of treatment,the scores of 4 items TCM syndromes of hypogastric pain,reduced appetite,depression and irritability and deep sigh in the two groups were significantly lower than those before treatment(all P<0.05).The proportion of peripheral blood CD 4+T lymphocyte subsets and the ratio of CD 4+/CD 8+T lymphocyte subsets in the two groups were significantly higher than those before treatment(all P<0.05),and the contents of interleukin(IL-8)and tumor necrosis factor-α(TNF-α)in drainage fluid in the two groups were significantly lower than those before treatment(all P<0.05),and the changes in observation group were significantly greater than those in control group(all P<0.05).Conclusion:Xiaoyao Loubei powder for patients after radical mastectomy can improve the clinical efficacy,relieve the related TCM symptoms,effectively regulate T lymphocytes,and reduce the inflammatory response in drainage fluid.
