类风湿关节炎患者血清IL-13水平表达及对外周血单个核细胞中Th17细胞亚群分化的影响 被引量:6

摘要 目的:探讨类风湿关节炎(RA)患者血清IL-13表达及对RA患者外周血单个核细胞(PBMC)中Th17细胞亚群分化的影响。方法:通过酶联免疫吸附试验(ELISA)检测活动期RA患者(活动组,n=30)、稳定期RA患者(稳定组,n=30)和健康体检者(正常组,n=30)外周血血清白细胞介素-13(IL-13)表达水平,分析血清IL-13水平与RA疾病活动评分(DAS-28)、红细胞沉降率(ESR)及C反应蛋白(CRP)的相关性;提取RA患者和健康体检者PBMC(设为对照组),分离纯化人CD4^+T细胞并用IL-13分组干预,采用ELISA和RT-PCR方法检测其上清和细胞中IL-17A表达水平。结果:①相比正常组,RA患者血清IL-13表达显著升高(P<0.001),其表达水平与DAS28评分、ESR和CRP呈正相关(均P<0.001);②RA患者CD4^+T细胞经重组人白细胞介素13(rhIL-13)刺激后其上清中IL-17A蛋白及mRNA表达均显著降低(均P<0.001)。结论:IL-13表达水平与RA患者疾病严重程度和免疫炎性反应密切相关,可能通过抑制RA患者CD4^+T细胞向Th17分化减少IL-17A分泌而控制RA发展。 Objective:To investigate the expression of serum interleukin(IL)-13 in patients with rheumatoid arthritis and and its effect on differentiation of Th17 cell subsets in peripheral blood mononuclear cell(PBMC).Methods:Enzyme linked immunosorbent assay(ELISA)was used to detect the expression of serum IL-13 in patients with active RA(active group,30 cases),stable RA(stable group,30 cases)and healthy controls(normal group,30 cases).The relationship between serum IL-13 level and disease activity score(DAS28),erythrocyte sedimentation rate(ESR)and C-reactive protein(CRP)was analyzed.PBMCs were isolated from RA patients and healthy people,and human CD4^+T cells were isolated and purified.IL-13 was divided into groups for intervention.The expression level of IL-17A in supernatant and cells was detected by ELISA and RT-PCR.Results:Compared with the normal group,serum IL-13 expression level in RA patients was significantly increased(P<0.001),and its expression level was positively correlated with DAS28 score,ESR and CRP(all P<0.001).The expression of IL-17A protein and mRNA in the supernatant of CD4^+T cells in RA patients after stimulation with rhIL-13 was significantly decreased(all P<0.001).Conclusion:The expression level of IL-13 is closely related to the disease severity and immune inflammatory response of RA patients,which may control the development of RA by inhibiting the differentiation of CD4^+T cells to Th17 and reducing the secretion of IL-17A.
