输尿管狭窄治疗中几个关键问题的探讨 被引量:16

Discussion on several key issues in the treatment of ureteral stricture

摘要 输尿管狭窄的病因、狭窄部位、狭窄长度及程度等多种变量决定了狭窄病情的复杂性、治疗方法的多样性、处理难度的挑战性。当前国内外尚无可借鉴的输尿管狭窄诊疗指南,且很多手术方式适应证的界定相对模糊,疗效判定也缺乏统一标准,使临床医生面对输尿管狭窄的处置往往较为畏惧。而另一方面,近年来随着新技术的开展,输尿管狭窄的诊疗有了长足进步,机器人辅助腹腔镜、游离移植物镶嵌、阑尾代输尿管等技术的兴起使治疗理念发生重要转变。本文将围绕输尿管狭窄诊疗策略的发展与变迁进行简要剖析,重点探讨输尿管狭窄治疗过程中的争议问题。 Various factors such as etiology,location,length and degree of ureteral stricture,determine the complexity of the situation,the diversity of treatments,and the difficulty of handling.At present,there is no guideline for the diagnosis and treatment of ureteral stricture.The indications for different operations are vague and evaluation criteria of clinical efficacy of surgical treatment is not uniform,which makes urologists more afraid of the treatment of ureteral stricture.On the other hand,with the development of new technologies in recent years,the diagnosis and treatment of ureteral stricture have made great progress.The emergence of robot-assisted laparoscopy,buccal mucosa graft onlay,and appendiceal ureteral replacement has brought great changes in the treatment concept of ureteral stricture.This article will briefly analyze several problems in the treatment of ureteral stricture,focusing on the controversy.
