摘要
目的:观察含与不含白芥子的敷贴对肺癌咳嗽的疗效及皮肤不良反应,为改良穴位敷贴提供依据。方法:收集2018年5月至2019年3月上海中医药大学附属岳阳中西医结合医院肺癌脾虚痰湿型咳嗽患者64例。对照组使用含白芥子的敷贴,试验组使用替换白芥子为延胡索的敷贴。每周3次,每次4~6h,治疗3周。对比皮肤过敏反应、咳嗽喘促症状、中医证候评分、免疫指标等。结果:治疗后试验组皮肤过敏反应积分小于对照组(P<0.05)。两组改善夜间咳嗽、气喘症状的效果相仿,但对照组对日间咳嗽的疗效优于试验组(P<0.05)。两组改善中医证候评分的效果相仿。治疗后对照组Ts细胞表达较治疗前降低(P<0.05)。结论:含白芥子的对照组皮肤不良反应较试验组剧烈,但在日间咳嗽的缓解情况及抑制Ts细胞方面均优于试验组,这可能与白芥子增加药物透皮率有关。提示医生需权衡疗效与皮肤反应选择药物。
Objective:To observe the effect and skin adverse reactions of lung cancer patients with cough by using acupoint application with and without Sinapis alba Linnaeus,and provide a basis for improving acupoint application.Methods:A total of 64 patients with lung cancer and cough were collected from Yueyang Hospital of Integrated Traditional Chinese and Western Medicine,Shanghai University of Traditional Chinese Medicine from May,2018 to March,2019.The control group used a Sinapis alba Linnaeus-containing applicator,and the test group used an applicator that replaced Sinapis alba Linnaeus with corydalis.Treated 4~6 h/time,three times a week for 3 weeks.Skin adverse reactions,cough and asthma,cough VAS scores,TCM syndrome scores,and immune indicators were compared.Results:The skin adverse reactions in the test group were lesser than those in the control group(P<0.05).The effects of nighttime cough and asthma were similar in the two groups,but the control group was better than the experimental group in the treatment of daytime cough(P<0.05).The effects of improving TCM syndrome score were similar in the two groups.The control group inhibited the expression of Ts cells(P<0.05).Conclusion:The control group is more severe than the test group in skin adverse reaction,but control group is better than the experimental group in relieving daytime cough and inhibiting Ts cells,which may be related to the increase of drug transdermal rate by Sinapis alba Linnaeus.Doctors need to weigh the efficacy and skin adverse reaction when choose drugs.
