高通量血液透析治疗老年尿毒症患者钙磷代谢异常的临床疗效 被引量:5

Clinical Efficacy of High Flux Hemodialysis in the Treatment of Elderly Uremic Patients with Abnormal Calcium and Phosphorus Metabolism

摘要 目的探讨高通量血液透析治疗老年尿毒症患者钙磷代谢异常的临床疗效。方法将2018年8月至2019年12月于我院进行治疗的80例钙磷代谢异常的老年尿毒症患者作为研究对象,按其入院顺序,单数为低通量组,双数为高通量组,每组40例。低通量组采用低通量血液透析治疗,高通量组采用高通量血液透析治疗。比较两组透析后血磷、血钙、血肌酐、尿素氮以及β2微球蛋白及可溶性转铁蛋白受体(sTfR)、甲状旁腺激素(PTH)、碱性磷酸酶(AKP)水平变化情况,同时比较两组临床疗效。结果高通量组血磷、血钙、血肌酐、尿素氮以及β2微球蛋白、sTfR、PTH、AKP水平均明显低于低通量组(P<0.05);高通量组治疗总有效率与低通量组比较差异无统计学意义(P>0.05)。结论在老年尿毒症患者钙磷代谢异常的治疗中,高通量血液透析治疗与低通量血液透析治疗在临床治疗效果上无明显差异,但应用高通量血液透析治疗能进一步改善机体内钙磷代谢异常情况,能有效清除尿毒症患者体内的毒素分子,优化临床治疗效果。 Objective To investigate the clinical efficacy of high flux hemodialysis in the treatment of elderly uremic patients with abnormal calcium and phosphorus metabolism.Methods Eighty elderly uremic patients with abnormal calcium and phosphorus metabolism who were treated in our hospital from August 2018 to December 2019 were selected as the research objects.According to the order of admission,the odd number was low flux group and the even number was high flux group,40 cases in each group.The low flux group was treated with low flux hemodialysis,and the high flux group was treated with high flux hemodialysis.The level changes of serum phosphorus,calcium,creatinine,urea nitrogen,β2-microglobulin,soluble transferrin receptor(sTfR),parathyroid hormone(PTH)and alkaline phosphatase(AKP)were compared between the two groups,simultaneously compare the clinical efficacy of the two groups.Results The levels of serum phosphorus,calcium,creatinine,urea nitrogen,β2-microglobulin,sTfR,PTH and AKP in the high flux group were significantly lower than those in the low flux group(P<0.05);there was no significant difference in the total effective rate between the high flux group and the low flux group(P>0.05).Conclusion In the treatment of abnormal calcium and phosphorus metabolism in elderly patients with uremia,there is no significant difference between high flux hemodialysis and low flux hemodialysis in clinical treatment effect,but the application of high flux hemodialysis treatment can further improve the abnormal calcium and phosphorus metabolism in the body,effectively remove the toxin molecules in uremic patients,and optimize the clinical treatment effect.
