摘要
目的评价经皮穴位电刺激改善膝关节置换术术后疼痛的有效性及安全性。方法收集2018年4月—2019年9月在中国中医科学院望京医院行人工全膝关节置换术的患者98例,随机分为经皮穴位电刺激(TEAS)组和对照组各49例。对照组采用基础术后镇痛治疗,TEAS组采用经皮穴位电刺激+基础镇痛治疗,观察比较2组术前术后各时点膝关节VAS评分、痛阈值和HSS评分,评定临床疗效。结果2组术前及术后第1天膝关节VAS评分及痛阈值比较差异均无统计学意义(P均>0.05);术后第3天及术后第7天TEAS组的膝关节VAS评分均明显低于对照组(P均<0.05),而痛阈值均明显高于对照组(P均<0.05)。2组术前及术后第14天膝关节HSS评分比较差异均无统计学意义(P均>0.05),术后第7天TEAS组的膝关节HSS评分明显高于对照组(P<0.05)。结论经皮穴位电刺激治疗可以作为全膝关节置换术后多模式镇痛的组成部分,可以明显减轻患者术后疼痛,促进术后膝关节功能早期康复。
Objective It is to evaluate the effectiveness and safety of transcutaneous electrical acupoint stimulation(TEAS)in improving pain after knee arthroplasty.Methods A total of 98 patients who underwent total knee arthroplasty in Wangjing Hospital from April 2018 to September 2019 were collected and randomly divided into TEAS group and control group with 49 cases in each group.The control group was treated with basic postoperative analgesia,and the TEAS group was treated with TEAS+basic analgesia.The VAS score,pain threshold and HSS score of knee joint were observed and compared before surgery and at each time point after surgery,the clinical efficacy of the two groups was evaluated.Results There was no significant difference in VAS scores and pain threshold of knee joint before and on the first day after surgery between the two groups(P>0.05);the VAS scores in the TEAS group were significantly lower on the 3rd and 7th postoperative days than those in the control group,and the pain threshold was significantly higher than the control group(P<0.05).There was no significant difference in HSS scores before and on the 14th day after surgery between the two groups(P>0.05),the HSS score of the TEAS group was significantly higher than that of the control group on the 7th day after surgery(P<0.05).Conclusion TEAS can be used as a component of multi-modal analgesia after total knee arthroplasty,which can reduce postoperative pain scores and promote early recovery of postoperative knee joint function.
