摘要
由于急性创伤、慢性骨骼肌肉系统疼痛等问题,存在躯体和社会功能部分受损的骨科患者合并精神卫生问题及精神障碍的比例较高。精神卫生问题及精神障碍对骨科患者手术效果有直接影响。如何尽早发现精神卫生问题及精神障碍的临床症状,并进行科学的初步检查、评估和处理是骨科加速康复面临的重要临床问题。为了进一步推广骨科加速康复围手术期精神卫生问题及精神障碍的评估与管理工作,提高医疗质量和安全,国家卫生健康委加速康复外科专家委员会骨科专家组、中国研究型医院学会骨科加速康复专业委员会、中国康复技术转化及促进会骨科加速康复专业委员会联合相关学科专家成立骨科加速康复系列共识编写专家委员会,以临床问题为导向,遵循循证医学原则,归纳出3个方面的问题:(1)骨科患者合并精神卫生问题及精神障碍的分类与共性表现特征;(2)骨科患者围手术期精神卫生问题及精神障碍的临床表现、筛查和评估;(3)骨科患者围手术期精神卫生问题及精神障碍的治疗。按照上述3个方面的问题进行文献检索和归纳,通过线上结合线下会议反复讨论和修改后形成本共识,期望其为我国骨科加速康复围手术期精神卫生问题及精神障碍的评估与管理的多学科协作起到积极的推动作用,最终达到加速患者康复的目的。
Due to acute trauma and chronic musculoskeletal pain,the proportion of mental health problems and mental disorders in orthopaedic patients with partially impaired physical and social functions is high.Mental health problems and mental disorders have a direct impact on the effect of orthopaedic surgery.It is important in clinical practice to detect mental health problems and clinical symptoms of mental disorders as early as possible and carry out scientific preliminary examination,evaluation and treatment in enhanced recovery after surgery(ERAS)program in orthopaedics.In order to promote the assessment andmanagement of perioperativemental health problems andmental disorders in orthopaedics,Orthopaedic ExpertGroup of ERAS profession Committee ofNationalHealth Commission,Orthopaedic ERAS profession Committee of Chinese Research Hospital Association,Orthopaedic ERAS profession Committee of China Association of Rehabilitation Technology Transformation and Promotion,together with experts from related disciplines,established a collaborative expert committee for this consensus writing.Focusing on the clinical problems and following evidence-based medicine principles,the experts summarized the following three aspects:(1)characteristics of mental health problems and mental disorders in orthopaedic patients;(2)clinicalmanifestations and assessment of perioperativemental health problems andmental disorders in orthopaedic patients;(3)treatment of mental health problems and mental disorders in patients undergoing orthopaedic surgery during perioperative period.Around these three aspects,literature was searched and induced.Through repeated discussions and revisions on line together with off line,this consensus was developed hoping to promote the multidisciplinary cooperation and realize repaid recovery for patients.
