摘要
目的探讨外周血中未成熟血小板分数(IPF)、白细胞分类在自身免疫性肝炎患者中的临床运用价值。方法85例自身免疫性肝炎患者作为本院2017年6月-2018年6月研究对象,其中活动期40例、缓解期45例,并以同期健康体检者40例作为对照组,对比不同病情患者外周血中白细胞分类计数、未成熟血小板分数,采用Pearson或Spearman相关性分析探讨未成熟血小板分数、白细胞分类与自身免疫性肝炎患者临床特点的关系,Logistic回归分析探讨患者肝功能不全的独立危险因素。结果活动期患者中性粒细胞/淋巴细胞比值、IPF比例明显高于缓解期、对照组患者(P<0.05),N/L、IPF与ALT、AST、TB、ANA滴度、病情活动度均呈正相关(P<0.05)ANA滴度、N/L、IPF均是患者肝功能不全的独立危险因素(P<0.05)。结论未成熟血小板分数和中性粒细胞/淋巴细胞与自身免疫性肝炎的病情及预后密切相关。
Objective To explore the clinical value of immature platelet fraction(IPF)and leukocyte classification in peripheral blood in patients with autoimmune hepatitis.Methods A total of 85 patients with autoimmune hepatitis were studied in our hospital from June 2017 to June 2018.Among them,40 patients were in active phase and 45 patients in remission phase.40 patients in the same period were taken as control group.Leukocyte count and immature platelet fraction in peripheral blood of patients with different conditions were compared.Pearson or Spearman correlation analysis was used to explore immature platelet fraction and white blood cell fraction.The relationship between cell classification and clinical characteristics of patients with autoimmune hepatitis was analyzed by Logistic regression analysis to explore the independent risk factors of liver dysfunction.Results The ratio of neutrophil to lymphocyte(%)and IPF(%)in active patients were 4.9±1.3 and 21.5±2.5 respectively,which were significantly higher than those in remission and control groups.N/L and IPF were positively correlated with ALT,AST,TB,ANA titers and disease activity(P<0.05).ANA titers,N/L,IPF were all independent risk factors for liver dysfunction(P<0.05).Conclusion Immature platelet fraction and neutrophil/lymphocyte are closely related to the condition and prognosis of autoimmune hepatitis.
