老年慢性阻塞性肺疾病患者肺功能及呼吸困难程度与体质量指数的相关性分析 被引量:8

Correlation analysis of pulmonary function and dyspnea and body mass index in elderly patients with chronic obstructive pulmonary disease

摘要 目的分析老年慢性阻塞性肺疾病(COPD)患者肺功能及呼吸困难程度与体质量指数(BMI)的相关性。方法选取2017年10月-2019年10月在成都市郫都区人民医院治疗的133例稳定期COPD老年患者为样本,开展横断面研究,采集患者基本资料,测量BMI、用力肺活量(FVC)、第1秒呼气容积占预计值百分比(FEV1%)、FEV1/FVC和6 min步行试验(6MWT)距离等指标,同时采用改良版英国医学研究委员会呼吸问卷(mMRC)评估患者呼吸困难程度。根据BMI将患者分为低BMI组(n=34)、正常组(n=58)和超重组(n=41),比较3组各项检查结果差异,并分析其与BMI的相关性,研究BMI对患者肺功能和呼吸困难程度的影响。结果低BMI组病程明显长于正常组和超重组(P<0.05);FVC和FEV1%明显低于正常组和超重组(P<0.05);3组6MWT和mMRC呼吸困难程度差异均有统计学意义(P<0.05);低BMI组呼吸困难程度明显高于正常组和超重组(P<0.05);COPD患者BMI与FVC、FEV1呈明显正相关性(P<0.05),与mMRC分级呈负相关性(P<0.05);低BMI组FVC、FEV1和6MWT检测值明显降低(P<0.05),而mMRC分级明显增加(P<0.05),超重组6MWT明显降低(P<0.05)。结论老年COPD患者BMI与肺功能、呼吸困难紧密相关,且低BMI和高BMI均可引起患者运动耐量下降,因此,维持正常体重对缓解病情极为重要。 Objective To study the correlation between lung function and dyspnea degree and body mass index(BMI)in elderly patients with chronic obstructive pulmonary disease(COPD).Methods A cross-sectional study was conducted on133 elderly patients with stable COPD treated in people’s hospital of Pidu district from October 2017 to October 2019.The basic data of the patients were collected in detail,and the BMI,forced vital capacity(FVC),forced expiratory volume in 1 s as a percentage of predicted value(FEV1%),FEV1/FVC and 6 min walking test(6 MWT)distance of all patients were measured.The modified British Medical Research Council Respiratory Questionnaire(m MRC)was used to assess the degree of dyspnea.According to BMI,patients were divided into low BMI group(n=34),normal group(n=58)and overweight group(n=41).The differences in the examination results were compared among the three groups,and the effects of BMI on lung function and dyspnea degree were studied.Results The course of disease of the patients in low BMI group was significantly longer than that in other two group(P<0.05),while the FVC and FEV1%were significantly lower than those in other two group(P<0.05),and there were statistically significant differences in the 6 MWT and m MRC dyspnea degree among the three groups(P<0.05).The degree of dyspnea of the patients in the low BMI group was more serious than that in other two group(P<0.05).Among COPD patients,BMI was significantly positively correlated with FVC and FEV1(P<0.05),and was negatively correlated with m MRC grading(P<0.05).FVC,FEV1 and 6 MWT in low BMI group reduced significantly(P<0.05),while m MRC grading increased significantly(P<0.05),and the 6 MWT in overweight group decreased significantly(P<0.05).Conclusions BMI is closely related to lung function and dyspnea in elderly patients with COPD and both low BMI and high BMI can cause the decrease of exercise tolerance.Therefore,maintaining normal weight is very important to relieve the disease.
