摘要
目的:观察凉血解毒化瘀汤对乙肝相关性慢加急性肝衰竭(HBV-ACLF)患者肝功能、凝血指标的影响。方法:选取98例HBV-ACLF患者,按随机数字表法分为对照组和观察组各49例。对照组采用常规西药治疗,观察组在对照组的基础上联合凉血解毒化瘀汤治疗。比较2组治疗前后肝功能、凝血指标、并发症以及临床疗效。结果:观察组总有效率为89.80%,高于对照组71.43%(P<0.05)。治疗后,2组谷丙转氨酶(ALT)、谷草转氨酶(AST)、血清总胆红素(TBil)和凝血酶原时间(PT)水平较治疗前降低,白蛋白(Alb)和凝血酶原活动度(PTA)较治疗前升高(P<0.05);观察组ALT、AST、TBil和PT水平低于对照组,Alb和PTA水平高于对照组(P<0.05)。治疗后,2组中医证候评分较治疗前降低(P<0.05),且观察组中医证候评分低于对照组(P<0.05)。观察组并发症发生率为6.12%,低于对照组28.57%(P<0.05)。结论:凉血解毒化瘀汤治疗HBV-ACLF患者,能够改善肝功能和凝血功能,降低并发症发生率,疗效肯定。
Objective:To observe the effect of Liangxue Jiedu Huayu tang on liver function and coagulation indexes in patients with hepatitis B virus-associated acute-on-chronic liver failure(HBV-ALCF). Methods:A total of 98 cases of HBV-ACLF patients were divided into the control group and the observation group according to the random number table method,49 cases in each group. The control group was given routine western medicine, and the observation group was additionally treated with Liangxue Jiedu Huayu tang based on the treatment of the control group. The liver function,coagulation indexes and complications before and after treatment as well as the clinical effect in the two groups were compared. Results: The total effective rate was 89.80% in the observation group, higher than that of 71.43% in the control group(P<0.05). After treatment, the levels of alanine aminotransferase(ALT), aspartate aminotransferase(AST),total bilirubin(TBil) and prothrombin time(PT) in the two groups were decreased when compared with those before treatment,and the levels of albumin(Alb) and prothrombin activity(PTA) were increased(P<0.05);the levels of ALT,AST,TBil and PT in the observation group were lower than those in the control group,and the levels of Alb and PTA were higher(P<0.05). After treatment, Chinese medicine syndrome score in the two groups were decreased when compared with those before treatment(P<0.05),and the score in the observation group was lower than that in the control group(P<0.05). The incidence of complications was 6.12% in the observation group,lower than that of 28.57% in the control group(P<0.05). Conclusion:Liangxue Jiedu Huayu tang for HBV-ACLF patients can improve liver function and coagulation function,and reduce incidence of complications,with definite curative effect.
