摘要
目的系统评价BMI与2型糖尿病(T2DM)患者死亡风险的相关性。方法检索PubMed、Embase等6个数据库,查找相关文献,研究类型为队列研究。检索时限为各数据库建库至2019年5月。由2名评价者独立对纳入研究进行筛选和资料提取,采用NOS量表对纳入文献进行质量评价,运用RevMan5.3软件对纳入文献进行Meta分析。结果最终纳入17项研究,共267 515例患者。Meta分析结果显示:与正常体重人群相比,超重人群T2DM死亡风险较低,差异有统计学意义[HR=0.90,95%CI(0.80,0.97),P=0.008];肥胖人群T2DM死亡风险与正常体重人群差异无统计学意义[HR=0.91,95%CI(0.81,1.01),P=0.07];年龄≥65岁肥胖人群死亡风险与正常人群比较,差异有统计学意义[HR=0.76,95%CI(0.68,0.85),P<0.001]。结论较正常体重人群而言,超重人群、年龄≥65岁肥胖T2DM人群死亡风险降低,可能是其保护因素。
Objective To systematically evaluate the correlation between body mass index and mortality risk of type 2 diabetes mellitus(T2DM).Methods A total of 6 databases including PubMed and Embase were searched to find relevant literatures and the research type was cohort study.The search time limit was from the establishment of every database to May 2019.The included studies were screened and the data were extracted by two researchers independently.The Newcastle-Ottawa Scale(NOS)was used to evaluate the quality of the included literatures,and RevMan5.3 software was used to conduct Meta-analysis of the included literatures.Results A total of 17 studies involving 267515 patients were enrolled into the analysis.The Meta-analysis results showed that compared with the normal weight population,the mortality risk of T2DM was obviously lower in the overweight patients,and the difference was statistically significant[HR=0.90,95%CI(0.80,0.97),P=0.008].There was no statistically significant difference in the mortality risk of T2DM between obese population and normal weight population[HR=0.91,95%CI(0.81,1.01),P=0.07];there was statistically significant difference in the mortality risk of obese people aged≥65 years and normal population.Conclusions Compared with people with normal weight,overweight people and obese people with T2DM aged over or equal to 65 years have a lower mortality risk,which may be protective factors.
