重症监护室医护人员MRSA鼻腔定植感染情况调查 被引量:1

摘要 目的对本院重症监护室(Intensive Care Unit,ICU)医护人员耐甲氧西林金黄色葡萄球菌(MRSA)鼻腔定植感染情况进行调查,探讨MRSA耐药性及相关防控对策。方法将2017年10月至2019年4月在ICU工作的152位医护人员作为MRSA感染研究对象,采集其鼻前庭拭子标本进行MRSA菌培养与分离,监测MRSA鼻腔定植感染情况,并进行耐药性分析。结果152位医护人员中,检出MRSA鼻腔定植感染9例,感染率为5.92%;MRSA对头孢西丁、苯唑西林、阿莫西林/克拉维酸、青霉素G、氨苄西林有高度耐药性,耐药百分比均为100%;但对万古霉素、替考拉宁、利奈唑胺及甲氧苄啶无耐药性。结论ICU医护人员存在MRSA鼻腔定植感染风险,且MRSA对部分常用抗生素有高度耐药性;加强对医护人员MRSA鼻腔定植感染的筛查,根据药敏实验结果合理选择抗菌药物,以降低MRSA医源性传播。 Objective To investigate the nasal colonization of methicillin-resistant Staphylococcus aureus(MRSA)in intensive care unit(ICU),and to explore the drug resistance and prevention and control strategies of MRSA.Methods A total of 152 medical staff working in ICU from October 2017 to April 2019 were selected as the objects.Their nasal vestibular swab specimens were collected for MRSA culture and isolation to monitor the MRSA nasal colonization and conduct drug resistance analysis.Results There were 9 cases of MRSA nasal colonization detected in the 152 objects,and the infection rate was 5.92%.MRSA were highly resistant to cefoxitin,oxacillin,amoxicillin/clavulanic acid,penicillin G,ampicillin,and the drug resistance percentage were all 100%.However,MRSA were not resistant to vancomycin,teicoplanin,linezolid and trimethoprim,and the resistance rate were all 0.00%.Conclusion ICU medical staff is at risk for MRSA nasal colonization,and MRSA are highly resistant to some common antibiotics.Intensive active screening of MRSA nasal colonization in ICU medical staff should be conducted based on the results of drug sensitivity tests,so as to reduce the iatrogenic transmission of MRSA.
