摘要
目的:探讨鼻内镜手术和鼻腔开放手术在鼻窦炎鼻息肉患者鼻腔梗阻症状及嗅觉功能中的临床应用效果.方法:选取本院于2017年1月-2018年6月收治的82例鼻窦炎鼻息肉患者为研究对象,按照随机数字法分为两组,每组各41例.对照组予以鼻腔开放手术治疗,研究组予以鼻内镜手术治疗.比较两组患者临床疗效、鼻腔最小横截面积(NMCA)、NMCA到前鼻孔距离(DCAM)、鼻腔容积(NCV)、鼻气道总阻力、嗅觉功能以及并发症发生情况.结果:研究组患者临床总有效率97.56%显著高于对照组78.05%(P<0.05).两组患者干预前DCAM、NMCA、NCV水平、鼻气道总阻力、嗅觉功能评分、鼻腔气道阻力正常及嗅觉功能正常比例比较,差异均无统计学意义(P>0.05);干预后两组DCAM水平、鼻气道总阻力及嗅觉功能评分均较干预前显著降低,NMCA、NCV水平、鼻腔气道阻力正常及嗅觉功能正常比例均较干预前显著升高,且研究组DCAM水平、鼻气道总阻力及嗅觉功能评分低于对照组,NMCA、NCV水平、鼻腔气道阻力正常及嗅觉功能正常比例高于对照组(P<0.05).研究组并发症发生率4.88%显著低于对照组19.51%(P<0.05).结论:鼻内镜手术相较于鼻腔开放手术而言,更显著改善鼻窦炎鼻息肉患者鼻腔梗阻症状及嗅觉功能,利于促进鼻腔结构恢复正常,疗效确切且并发症发生率低.
Objective:To investigate the clinical application of endoscopic sinus surgery and open nasal surgery in the symptoms of nasal obstruction and olfactory function in patients with sinusitis and nasal polyps.Method:A total of eighty-two patients with sinusitis and nasal polyps admitted to our hospital from January 2017 to June 2018 were selected as the research subjects.They were divided into two groups according to random number method,34 cases in each group.The control group was treated with open nasal surgery.The study group was treated with endoscopic sinus surgery.The clinical efficacy,nasal minimum cross-sectional area(NMCA),distance from NMCA to anterior nostril(DCAM),nasal volume(NCV),total nasal airway resistance,olfactory function and complications were compared between the two groups.Result:The total clinical effective rate of the study group(97.56%)was significantly higher than that of the control group(78.05%)(P<0.05).The differences were not statistically significant in DCAM,NMCA,NCV level,total nasal airway resistance,olfactory function score,normal nasal airway resistance and the proportion of normal olfactory function between the two groups before intervention(P>0.05).After intervention,DCAM level,total nasal airway resistance and olfactory function score of the two groups were significantly lower than those before intervention.NMCA,NCV level,normal nasal airway resistance and normal proportion of olfactory function were significantly lower than those before intervention.DCAM level,total nasal airway resistance and olfactory function score in the study group were lower than those in the control group,while NMCA,NCV level,normal nasal airway resistance and normal olfactory function ratio in the study group were higher than those in the control group(P<0.05).The incidence of complications in the study group(4.88%)was significantly lower than that in the control group(19.51%)(P<0.05).Conclusion:Compared with open nasal surgery,endoscopic sinus surgery can significantly improve the symptoms of nasal obstruction and olfactory function in patients with sinusitis and nasal polyps.It is conducive to promoting the normal structure of nasal cavity.The curative effect is definite and the incidence of complications is low.
