摘要
目的探讨老年膀胱癌尿路造口患者在生活重构过程中不同阶段的体验。方法采用目的抽样法,对10例老年尿路造口患者进行半结构式访谈,运用Colaizzi现象学分析法分析资料、提炼主题。结果老年尿路造口患者在生活重构中不同阶段的体验可以概括为以下5个主题。(1)负性心理期:恐惧、孤独和负担感。(2)觉醒期:偶然契机带来的转变。(3)渴望期:生命再认知。(4)适应期:社会心理适应。(5)稳定期:创伤后成长。结论老年尿路造口患者生活重构过程中存在阶段性的动态体验和复杂心理状态。护士需根据患者不同阶段的生理、心理认知状态和需求,探索并提供完善的延续护理,帮助患者早日完成生活重建。
Objective To explore the experience of life reconstruction of older bladder cancer patients with urinary stoma.Methods Ten older patients with urinary stoma were interviewed with semi-structured questions.Colaizzi analysis was used to analyze the data and summarize the themes.Results The experience of older adults with urinary stoma was summarized as five main themes:Negative psychological period(sense of fear,loneliness,and burden);awakening period(change brought by chance);desiring period(life recognition);adaptation period(psychosocial adaptation);stable period(post-traumatic growth).Conclusion The older patients with urinary stoma have serious psychological pressure in the process of life reconstruction.Nurses can explore and provide tailored continuous care according to patients’physiological and mental state and needs in different stages,so as to help patients complete life reconstruction as soon as possible.
