摘要
目的:分析2型糖尿病合并恶性肿瘤患者心理状况、应对方式及生存质量的现况,并探讨患者心理健康、应对方式与生存质量三者之间的关系。方法:本研究为横断面研究,选取2型糖尿病合并恶性肿瘤患者(恶性肿瘤在糖尿病之后或同时确诊)310例,采用症状自评量表(Sympton Checklist-90,SCL-90)、医学应对问卷(Medical Coping Modes Questionnaire,MCMQ)和世界卫生组织生存质量测定量表简表(World Health Organization Quality of Life Brief Version,WHOQOL-BREF)分别对患者的心理健康状况、应对方式和生存质量状况进行测评。结果:2型糖尿病合并恶性肿瘤患者SCL-90自评量表自评总分为(179.8±51.2)分,平均分≥2.2分的前3位的健康因子分别为:躯体化症状、焦虑、抑郁。患者应对方式的三个因子得分从低到高依次为:屈服、回避和面对因子。2型糖尿病合并恶性肿瘤患者与正常对照组相比环境和精神支柱与个人信仰得分高于正常对照组(P<0.05),在生理、心理、独立性、社会关系和总的生存质量与健康状况得分低于对照组(P<0.05)。2型糖尿病合并恶性肿瘤患者屈服因子与总体生存质量和健康状况、心理领域得分呈负相关;应对方式中,面对因子与总体生存质量和健康状况、生理领域、心理领域得分呈正相关;SCL-90总分与患者总体生存质量、生理领域、心理领域得分呈负相关。结论:2型糖尿病合并恶性肿瘤患者心理问题主要体现在病情、负面情绪以及人际交往等方面,通常患者采用积极正面的方式,但患者生存质量仍然较低。
Objective:To analyze mental health,coping modes and quality of life of type 2 diabetes mellitus(T2 DM)patients with cancer,and study their relationships.Methods:This study is a cross-sectional survey.310 cases of type 2 diabetic patients with cancer were selected.They were assessed by Symptom Checklist-90(SCL-90),World Health Organization Quality of Life Brief Version(WHOQOL-BREF)and Medical Coping Modes Questionnaire(MCMQ).Results:The total SCL-90 score of T2 DM patients with cancer was 179.8±51.2.The average scores in somatization,anxiety and depression were greater than or equal to 2 points,which ranked top 3 in the rating.The three coping modes of patients were facing,avoiding and yielding from low to high.Scores of T2 DM patients with cancer were higher than those of normal control in physical health,psychological health,level of independence,social relationships and overall quality of life and general health(P<0.05);and those of the former were higher than those of the latter in environment and spirituality/religion/personal beliefs(P<0.05).For T2 DM patients with cancer,the coping mode of yielding was negatively correlated to overall quality of life and general health and psychological health in MCMQ;the coping mode of facing was positively correlated to overall quality of life and general health,physical health and psychological health in MCMQ;and the total score of SCL-90 was negatively correlated to overall quality of life and general health,physical health and psychological health in MCMQ.Conclusion:The main problems of T2 DM patients with cancer were mainly in physical conditions,negative emotions and interpersonal relationships,etc.Patients usually adopt a positive coping mode,but the quality of life of patients is still low.
