能谱CT虚拟平扫技术在头颈部CTA中的应用 被引量:4

Application of Virtual Non-contrast Images Technique of Energy Spectrum CT in Head and Neck CT Angiography

摘要 目的:探讨能谱CT虚拟平扫技术在头颈部CT血管造影(CTA)中的价值。方法:回顾性分析82例能谱模式下行头颈部CTA检查患者的影像数据及临床资料。对每例原始图像采用3种方法处理:虚拟平扫减影、常规数字减影和非减影处理,并对三组血管VR和MIP图像的质量及53例伴有钙化斑块及动脉瘤伴钙化病例的减影后重建图像,进行钙化斑块显示情况进行评分,记录扫描后患者辐射剂量。结果:三组图像质量评分两两间有统计学差异(P<0.01),从大到小依次为常规数字减影组3.65±0.44.虚拟平扫减影组3.10±0.59.非减影处理组1.99±0.60。虚拟平扫减影组钙化斑块显示评分(2.65±0.87)显著高于常规数字减影组(1.58±0.45;P<0.01)。虚拟平扫减影组的CTDLvol(38.26±9.24)mGy.DLP(447.40±25.77)mGy.cm和ED(1.39±0.80)mSv显著低于常规数字减影组的CTDLvol(48.37±9.26)mGy.DLP(873.74±49.43)mGy.cm和ED(2.71±0.15)mSv(P<0.01)。结论:将能谱CT虚拟平扫数据用于头颈部数字减影CT血管造影,有良好的CTA图像质量,并有更好的血管钙化斑块显示及辐射减少,具有一定的应用临床价值。 Purpose:To explore the value of virtual non-contrast images technique of energy spectrum CT in head and neck CT angiography(CTA).Methods:The imaging and clinical data of 82 patients who completed head and neck CTA in the gemstone spectral imaging mode were analyzed retrospectively.The original images of each case were processed with three different ways,virtual non-contrast subtraction,conventional digital subtraction and non-subtraction.The image quality of vascular VR and vascular MIP in the three groups and the calcification plaque display in the subtraction reconstruction images of 53 patients with calcification plaque and aneurysm with caleification were scored.The radiation dose of the patients was recorded after scan.Results:Image quality score:virtual non-contrast subtraction group was(3.10±0.59),conventional digital subtraction group was(3.65±0.44),non-subtraction treatment group was(1.99±0.60),the difference between pairwise scores of the three groups were with statistical signifcant(P<0.01).Calcified plaque display score:virtual non-contrast subtraction group was(2.65±0.87),conventional digital subtraction group was(1.58±0.45).The difference of calcified plaque display score between the two groups was with statistical significant(P<0.01).Radiation dose:virtual non-contrast subtraction group was with CTDLvol(38.26±9.24)mGy,DLP(447.40±25.77)mGy*cm,ED(1.3910.80)mSv,conventional digital subtraction group was with CTDLvol(48.37±9.26)mGy,DLP(873.74±49.43)mGy*cm,ED(2.71±0.15)mSv.The difference of radiation dose between the two groups was with statistical significant(P<0.01).Conclusion:It has certain clinical value to use virtual non-contrast images data derived from energy spectrum CT as head and neck digital subtraction CT angiography mask data.
