摘要
目的通过头颈部CT血管造影(CTA)观察颈动脉斑块易损性,分析其与眼底成像下视网膜血管管径的关系,探讨通过观察视网膜血管管径的改变评估颈动脉斑块易损性的可行性。方法回顾性研究临床考虑颈动脉粥样硬化并行头颈部CTA检查患者单眼及颈动脉1 246例。分段观察颈动脉斑块的易损性,将入选者分为3组:无斑块组(V0)423例,稳定性斑块组(V1)417例(钙化斑块)和易损性斑块组(V2)396例(非钙化斑块和/或混合斑块)。同期行眼底镜彩色照相,测量视网膜中央动脉管径(CRAE)和视网膜中央静脉管径(CRVE)。观察视网膜血管管径与颈动脉粥样硬化斑块易损性的关系。结果 CRVE与颈动脉粥样硬化斑块易损性呈正相关(B=0.005,P=0.016,OR=1.01);CRAE与颈动脉粥样硬化斑块易损性呈无相关性(B=-0.003,P=0.347,OR=1.00)。结论 CRVE增大与颈动脉粥样硬化斑块稳定性呈正相关,眼底镜彩色照相观察视网膜血管管径可以成为判断颈动脉粥样硬化的一个筛选方法。
Objective In this study, the vulnerability of carotid artery plaque was observed by CTA in the head and neck, and its relationship with the retinal vascular diameter was analyzed. The feasibility of assessing the vulnerability of carotid artery plaque by observing the changes of retinal vascular diameter was discussed. Methods 1 246 A retrospective study of 1246 patients with carotid atherosclerosis underwent head neck CTA examination. The vulnerability of carotid artery plaque was observed in segments. All the cases were devided into 3 groups： group V0, no plaque （423 cases）; group V1, stable plaque （calcified） （417 cases）; group V2, vulnerable plaque （non-calcified or/and mixed） （396 cases）. The diameters of the central retinal artery （CRAE） and the central retinal vein （CRVE） were measured through fundus photographyto find their relationship withthe vulnerability of carotid atherosclerotic plaque. Results CRVE was positively correlated with vulnerability of carotid atherosclerotic plaques （B=0.005, P =0.016, OR=1.01）, and CRAE was not correlated with vulnerability of carotid atherosclerotic plaques （B=-0.003, P =0.347, OR=1.00）. Conclusion The increase of CRVE lumen is positively correlated with the vulnerability of carotid atherosclerotic plaque. Retinal angiography can be a screening method for carotid atherosclerosis.
