摘要
目的:观察芍药汤联合英夫利昔单抗治疗中重度溃疡性结肠炎的临床疗效。方法:将68例中重度溃疡性结肠炎患者随机分为两组各34例。对照组予英夫利昔单抗治疗,治疗组在对照组基础上予芍药汤治疗,两组疗程均为3个月。结果:治疗后总有效率为94.12%,对照组为76.47%,两组比较差异具有统计学意义(P﹤0.05);两组患者治疗后腹痛、腹泻、黏液脓血便积分及Mayo评分、Baron评分均明显降低,且治疗组均明显低于对照组(均P﹤0.05);两组患者治疗后血小板计数、D二聚体、肿瘤坏死因子-α、白细胞介素-6水平等参数均显著降低(P﹤0.05),凝血酶原时间、白细胞介素-10水平均明显升高,且治疗组改善均优于对照组(均P﹤0.05)。结论:芍药汤联合英夫利昔单抗治疗中重度溃疡性结肠炎疗效肯定,能够显著改善患者症状,提高临床疗效,促进肠黏膜的恢复,可能与降低炎性反应并抑制高凝状态有关。
Objective:To observe the clinical effect of Shaoyao decoction and infliximab on moderate or severe ulcerative colitis.Methods:68 cases of patients with moderate or severe ulcerative colitis were randomly divided into two groups,34 cases in each group.The control group was treated with infliximab,while the treatment group was treated with Shaoyao decoction on the basis of the control group.For both groups,the course of treatment was 3 months.Results:After treatment:the total effective rate was 94.12% in the treatment group and 76.47% in the control group,and the comparative difference between two groups was statistically significant(P ﹤ 0.05);the scores of abdominal pain,diarrhea and mucopus bloody stools as well as Mayo score and Baron score of patients after treatment were significantly lowered in both groups,and the treatment group was significantly lower than the control group(all P ﹤ 0.05);the levels of indexes such as platelet count,D-dimer,tumor necrosis factor-αand interleukin-6,etc.of patients were significantly decreased in both groups(P﹤0.05),and the levels of prothrombin time and interleukin-10 were significantly increased,and the improvement in treatment group was better than in control group(all P ﹤ 0.05).Conclusion:The clinical effect of Shaoyao decoction and infliximab on moderate or severe ulcerative colitis is definite,which can significantly improve the symptoms of patients,enhance the clinical effect and promote the recovery of intestinal mucosa,which may be related to the combined application of infliximab and Shaoyao decoction,so as to reduce inflammatory reaction and inhibit hypercoagulable state.
