摘要
目的分析<35岁年轻乳腺癌患接受者去势治疗后生存质量的变化,探讨其影响因素。方法选取我院2015年1月至2017年6月收治的<35岁女性乳腺癌患者,在初诊时6、12、18、24个月时向患者发放EORTC-C30量表、EORTC-BR23量表,由患者独立完成量表调查。结果 87例患者接受问卷调查,6例失访。除认知功能外,EORTC-C30量表的各个维度在24个月时均能恢复至基线水平。EORTCBR23量表中,手臂症状、乳房症状和躯体形象的评分在24个月时不能恢复至基线水平,其余维度均能恢复至基线水平。多元回归分析提示乳房及腋窝淋巴结手术方式是影响整体生存质量最重要的因素(均P<0.05)。结论 <35岁年轻乳腺癌患者接受GnRH-a去势治疗总体生存质量良好,认知功能的下降需引起关注。保留乳房及保留腋窝的患者具有更好的生存质量。
Objective To analyze the tendency of quality of life in breast cancer patients younger than 35 years old undergone castration therapy,exploring its influencing factors.Methods The breast cancer patients younger than 35 years old were recruited from 2015 January to 2018 June in our center.The paper of quality of life questionnaire core⁃30(EORTC⁃C30)and questionnaire breast cancer module 23(EORTC⁃BR23)were issued to participants at first visit,6 months later,12 months later,18 months later and 24 months later.The questionnaires should be accomplished independently.Results The data from 87 patients were collected,6 patients were lost during follow up.According to EORTC⁃C30 questionnaire,the scores of all subscales resumed to baseline lever at 24 months except cognitive functioning.In line with the EORTC⁃BR23 questionnaire,the scores of subscales of arm symptoms,breast symptoms and body image could not resume to baseline at 24 months,whereas the remaining aspects can recover to baseline lever.Multiple regression analysis revealed that surgical method to breast lumps and axillary lymph nodes had the most important impact on global quality of life(P<0.05,respictively).Conclusion The breast cancer patients younger than 35 years old undergone castration therapy gain considerably well quality of life,meanwhile the decline of cognitive functioning should be concerned.The patients undergone breast conservative therapy and axillary conservative surgery had better quality of life.
