摘要
[目的]探讨我国三级甲等医院符合处方权申请资格的普外科护士可开具的纠正水、电解质、酸碱平衡紊乱药以及血容量扩张药、营养药物的处方内容及处方形式。[方法]基于文献调研法及半结构访谈法,拟定护士纠正水、电解质、酸碱平衡紊乱药以及血容量扩张药、营养药物处方权内容的研究专家函询问卷,运用德尔菲法对26名医疗及护理专家进行2轮函询论证,确定普外科护士可以开具的纠正水、电解质、酸碱平衡紊乱药以及血容量扩张药、营养药物处方内容。[结果]两轮专家函询问卷回收率分别为86.67%、100.00%,专家权威程度系数分别为0.85和0.86,初步确定了我国普外科护士可以开具的纠正水、电解质、酸碱平衡紊乱药及血容量扩张药、营养药物共25种,其中倾向独立处方5种,倾向协议处方11种,倾向独立/协议处方9种。[结论]医疗及护理专家建议符合处方权申请资格并经过培训、考核后的普外科护士可开具部分纠正水、电解质、酸碱平衡紊乱药和营养药,不建议开具血容量扩张药。
Objective:To probe into the contents and forms of prescription right of drugs that correcting imbalance of water,electrolyte and acid⁃base,dilatating blood volume and providing nutrition,and prescirbed by general surgical nurses who met applicant qualification of prescription right in tertiary grade A hospital in China.Methods:Based on the literature research method and semi⁃structured interview method,experts enquiry questionnaire for the contents of nurses’prescription right of drugs that correcting imbalance of water,electrolyte and acid⁃base,dilatating blood volume and providing nutrition,was preliminarily compiled.Using the Delphi method,two rounds of enquiries were conducted among 26 medical and nursing experts.Then,the contents of prescriptions for water,electrolytes,acid⁃base balance disorders,blood volume expansion drugs and nutritional drugs that can be prescribed by general surgery nurses were determined.Results:The questionnaire recovery rates of two rounds of expert enquiries were 86.67%and 100.00%,respectively.And authority coefficients of experts were 0.85 and 0.86,respectively.Among drugs that correcting imbalance of water,electrolyte and acid⁃base,dilatating blood volume and providing nutrition,totally 25 kinds of drugs that could be prescirbed by general surgical nurses in China,were preliminarily identified.Among them,there were 5 kinds of drugs tending to be listed as the form of independence prescription.There were 11 kinds of drugs tending to be listed as the form of cipher prescription.There were 9 kinds of drugs tending to be listed as the form of independence or cipher prescription.Conclusion:Medical and nursing experts suggested that general surgerical nurses who met the applicant qualification of prescription right,had been trained and passed examination,could prescribe part of drugs that correcting imbalance of water,electrolyte and acid⁃base,and providing nutrition.However,it was not recommended for general surgerical nurses to prescribe drugs that dilatating blood volume.
