摘要
目的构建卵巢癌患者BRCA基因检测知信行问卷,并检验其信效度,为医护人员开展现状调查提供依据。方法以知信行理论为框架,通过查阅国内外文献,小组讨论,并结合临床实践,采用德尔菲法,对来自不同领域的15名专家进行2轮专家咨询,形成初始问卷。选取2021年3~5月山东省肿瘤医院收治的241例卵巢癌患者进行语言调试,修订个别用词,同时进行项目分析及因子分析;6~8月的279例患者进行验证性因子分析,完成信效度检验。结果15名专家均完成了2轮专家咨询,判断依据(Ca)为0.900,熟悉程度(Cs)为0.813,权威系数(Cr)为0.857;2轮专家咨询Kendall’s W协调系数分别为0.187和0.155,P<0.001。构建的卵巢癌患者BRCA基因检测知信问卷包括3个维度36个条目,其中知识维度16个条目,态度/信念维度12个条目,行为维度8个条目。问卷的Cronbach’sα系数值为0.937,重测信度系数为0.948,内容效度指数(S-CVI)为0.941;探索性因子分析提取3个公因子,累积方差贡献率为55.029%;验证性因子分析显示模型拟合度良好,χ^(2)/df=1.297,RMSEA=0.033,SRMR=0.043,CFI=0.965,TLI=0.963,IFI=0.965。结论卵巢癌患者BRCA基因检测知信行问卷具有良好的信效度,可作为患者BRCA基因检测知信行水平的评估工具。
Objective To develop and test the reliability and validity of the BRCA gene testing knowledge,attitude and practice(KAP)questionnaire for ovarian cancer patients,so as to lay the foundation for medical staff to conduct the status survey.Methods Based on the theory of KAP,Delphi method was used to conduct two rounds of expert consultation among 15 experts from different fields to form the initial questionnaire,through consultation of domestic and foreign literature,group discussion and combination of clinical practice.Totally 241 cases with ovarian cancer admitted to Shandong Cancer Hospital from March to May,2021 were collected for language debugging,revision of individual words,and item analysis and factor analysis;279 samples from June to August was performed for confirmatory factor analysis to complete the reliability and validity test.Results All 15 experts completed two rounds of expert consultation.The judgment basis(CA)was 0.900,the familiarity(CS)was 0.813,and the authority coefficient(CR)was 0.857.Kendall’s w coordination coefficient of two rounds of expert consultation was 0.187 and 0.155 respectively,P<0.001.The constructed BRCA gene testing KAP questionnaire for ovarian cancer patients includes 36 items in 3 dimensions,including 16 items in knowledge dimension,12items in attitude/belief dimension and 8items in behavior dimension.The Cronbach’sαcoefficient of the questionnaire was 0.937,the test-retest reliability was 0.948,the S-CVI was 0.941.Three factors were extracted by the exploratory factor analysis,and the cumulative contribution percentage of variance was 55.029%.The confirmatory factor analysis showed excellent goodness of fit index for the three-factor structure(χ^(2)/df=1.297,RMSEA=0.033,SRMR=0.043,CFI=0.965,TLI=0.963,IFI=0.965).Conclusions The KAP questionnaire of BRCAgene testing for ovarian cancer patients has good reliability and validity,and it can be used as a tool to evaluate the KAP level of BRCAgene testing in patients.
