摘要
目的观察止痛如神汤联合耳穴压丸用于环状混合痔术后的疗效。方法选择2017年1月—2018年12月在上海市金山区中西医结合医院行内套外剥保留齿线术治疗的60例环状混合痔术后患者,随机分为2组,对照组30例术后给予肛肠科常规换药处理,治疗组30例则在对照组术后处理的基础上给予止痛如神汤坐浴熏洗联合耳穴压丸治疗,2组均以7 d为1个疗程,治疗3个疗程。疗程结束后观察2组的临床疗效,记录2组创面愈合时间、术后各时间点肛门疼痛和肛缘水肿评分、水肿消退时间。结果治疗组临床疗效与对照组比较差异无统计学意义(P>0.05);治疗组创面愈合时间为(18.7±2.9)d,对照组愈合时间为(22.0±3.1)d,治疗组明显短于对照组(P<0.05);2组干预1,3,5,7d后VAS评分均比治疗前明显下降(P均<0.05),但治疗组VAS评分比同期对照组更低(P均<0.05);干预7 d后,治疗组肛缘水肿评分明显低于对照组,且水肿消退时间明显短于对照组(P均<0.05)。结论环状混合痔术后采用止痛如神汤坐浴熏洗联合耳穴压丸治疗,可以明显缓解术后肛门疼痛,促进肛缘水肿加速消退,从而促进创面愈合。
Objective It is to observe the curative effect of Zhitong Rushen decoction combined with auricular point pill pressing in the treatment of circular mixed hemorrhoids after operation.Methods Sixty patients with circular mixed hemorrhoids who underwent internal ligation and external dissection plus dentate line preserving suspend ligation from January 2017 to December 2018 in Jinshan District Hospital of Integrated Traditional Chinese and Western Medicine were selected and randomly divided into two groups,30 cases in each group.The control group was treated with routine dressing change in the department of anorectology after operation.The control group was treated with routine dressing change in the Department of anorectology after operation.The treatment group was treated with Zhitong Rushen decoction by sitz bath plus fumigation and ear acupoint pill pressing on the basis of the control group’s post-operative treatment.7 days as one course of treatment,both groups were treated for 3 courses.At the end of the course of treatment,the clinical efficacy,wound healing time,anal pain,anal margin edema score and edema subsidence time of the two groups were observed and recorded.Results There was no significant difference in the clinical efficacy between the treatment group and the control group(P>0.05).The healing time of wounds was 18.7+/-2.9 d in the treatment group,and 22.0+/-3.1 in the control group,the healing time of wounds in the treatment group was significantly shorter than that in the control group(P<0.05).The VAS scores of the treatment group and the control group after 1,3,5,7 days of treatment were significantly lower than those before treatment(all P<0.05),but the VAS scores of the treatment group were significantly lower than those of the control group(all P<0.05).After 7 days of treatment,the score of anal edema and the time of edema subsidence in the treatment group were significantly less than those in the control group(all P<0.05).Conclusion Zhitong Rushen decoction by sitz bath plus fumigation and ear acupoint pill pressing can significantly relieve postoperative anal pain,promote the rapid disappearance of anal edema,and thus promote wound healing after surgery for circular mixed hemorrhoids.
