摘要
目的评价耳穴埋豆联合亚甲蓝(Methylene Blue,MB)皮下注射对环状痔伴嵌顿术后疼痛的改善。方法收集2019年1月—2019年12月因环状痔伴嵌顿入住该院并行混合痔外剥内扎(Milligan-Morgan,M-M)术者60例,将其随机分成两组,对照组于手术结束前予以亚甲蓝混合液皮下点状注射;观察组在对照组基础上,于手术开始前1 h行耳穴埋豆治疗。观察两组患者手术之后第1天、第3天、第7天的疼痛视觉模拟评分法(Visual Analogue Scale/Score,VAS)评分、并发症发生率与疗效。结果分别比较手术之后第1、3、7天疼痛VAS值,观察组VAS值与对照组相比,均显著降低(P<0.05);术后疗效方面,对比两组间区别并不明显(P>0.05);在术后尿潴留、感觉异常与灼痛感方面,观察组与对照组相比,差异明显(P<0.05),而观察组创缘水肿为10%(3/30),虽明显低于对照组的16.67%(5/30),但无统计学意义(P>0.05)。结论耳穴埋豆联合亚甲蓝混合液相比单纯亚甲蓝混合液在环状嵌顿痔术后镇痛方面效果理想,有一定的临床应用价值。
Objective The postoperative analgesic effect of methylene blue(MB)combined with ear acupoints embedding on circular incarcerated hemorrhoids.Methods A total of 60 patients with circumferential hemorrhoids and incarcerated hemorrhoids admitted to our hospital from January to December 2019 were randomly divided into two groups.The control group was treated with MB by subcutaneous injection before operation.On the basis of the control group,the observation group was treated with ear acupoints embedding therapy 1 hour before operation.The visual analogue scale(VAS)score,complication rate and curative effect of the two groups were observed on the first day,the third day and the seventh day after operation.Results The pain VAS scores on the 1 st,3 rd and 7 th day after operation showed that the scores of the observation group were lower than those of the control group(all P<0.05).There was no statistically significant difference in postoperative treatment effects between the two groups(P>0.05).The incidence of postoperative urine retention,burning pain and paresthesia in the control group was higher than that in the observation group(P<0.05).Although the incidence of postoperative wound edema in the control group was higher than that in the observation group and it was not statistically significant(P>0.05).Conclusion Compared with pure MB,ear acupoint embedding combined with MB is more effective in analgesia after circular incarcerated hemorrhoids operation,and has certain clinical application value.
