摘要
目的脑卒中是临床常见病,并发症较多,脑卒中后焦虑抑郁是其中较为常见的一种。本研究旨在分析养血清脑颗粒治疗脑卒中后焦虑抑郁的效果以及对神经递质水平影响。方法选取2017-01-01-2018-06-01漯河市召陵区人民医院收治的93例卒中后焦虑抑郁患者为研究对象,按照组间基本特征均衡可比原则,分为对照组(46例)和观察组(47例)。对照组给予帕罗西汀口服,20mg/(次·d);观察组增加养血清脑颗粒口服,4g/次,3次/d,均治疗2个月。对比治疗后两组血清神经递质水平、焦虑抑郁情绪、神经功能、临床疗效及不良事件发生情况。结果观察组多巴胺(dopamine,DA)水平为(9.90±0.80)ng/L,高于对照组的(7.35±0.71)ng/L,t=16.245,P<0.001;去甲肾上腺素(norepinephrine,NE)水平为(88.03±7.10)ng/L,高于对照组的(70.03±6.11)ng/L,t=13.092,P<0.001;5-羟色胺(5-hydroxytryptamine,5-HT)水平为(401.21±39.85)ng/L,高于对照组的(309.68±36.68)ng/L,t=11.518,P<0.001。观察组汉密尔顿焦虑量表(Hamilton anxiety Scale,HAMA)评分为(4.31±0.55)分,低于对照组的(6.80±0.88)分,t=16.400,P<0.001;汉密顿抑郁量表(Hamilton depression scale,HAMD)评分为(4.41±0.38)分,低于对照组的(7.03±0.79)分,t=20.311,P<0.001;美国国立卫生研究院卒中量表(national institute of health stroke scale,NIHSS)评分为(8.91±1.16)分,低于对照组的(12.34±1.88)分,t=10.614,P<0.001。观察组总有效率为91.49%,高于对照组的73.91%,差异有统计学意义,χ^2=5.042,P=0.025;不良事件发生率为10.64%,低于对照组的13.04%,差异无统计学意义,χ^2=0.129,P=0.720。结论养血清脑颗粒能够有效调节卒中后焦虑抑郁患者神经递质水平,消除负性情绪,改善神经功能,安全有效。
OBJECTIVE Stroke is a common clinical disease with many complications.Post-stroke anxiety and depression is one of the more common.The purpose of this study was to analyze the effect of Yangxue Nao Granules in treating post-stroke anxiety and depression and the effect on neurotransmitter levels.METHODS A total of 93 patients with post-stroke anxiety and depression treated in People’s Hospital of Zhaoling District of Luohe City from January 1,2017 to June 1,2018 were selected as the research subjects.According to the principle of equal comparability of basic characteristics between groups,they were divided into control group(46 cases)and observation group(47 cases).The control group was given paroxetine orally,20 mg/(times·day),while the observation group was given Yangxuenao granules orally,4 g/times,3 times/d.Both groups were treated for 2 months.The serum neurotransmitter levels,anxiety and depression,neurological function,clinical efficacy and adverse events were compared between the two groups after treatment.RESULTS The dopamine(DA)level in the observation group was(9.90±0.80)ng/L,which was higher than that in the control group(7.35±0.71)ng/L,t=16.245,P<0.001.The level of norepinephrine(NE)was(88.03±7.10)ng/L,which was higher than the control group(70.03±6.11)ng/L,t=13.092,P<0.001.The level of 5-Hydroxytryptamine(5-HT)was(401.21±39.85)ng/L,higher than the control group(309.68±36.68)ng/L,t=11.518,P<0.001.The Hamilton Anxiety Scale(HAMA)score of the observation group was(4.31±0.55)points,lower than the control group’s(6.80±0.88)points,t=16.400,P<0.001;Hamilton Depression Scale(HAMD)score is(4.41±0.38)points,lower than the control group(7.03±0.79)points,t=20.311,P<0.001;National Institute of Health stroke scale(NIHSS)the score was(8.91±1.16)points,which was lower than the control group’s(12.34±1.88)points,t=10.614,P<0.001.The total effective rate in the observation group was 91.49%,which was higher than 73.91%in the control group,the difference was statistically significant,χ^2=5.042,P=0.025;the incidence of adverse events was 10.64%,which was lower than 13.04%in the control group the difference was not statistically significant.χ^2=0.129,P=0.720.CONCLUSION Yangxuenao Granules can effectively regulate neurotransmitter levels in post-stroke anxiety and depression patients,eliminate negative emotions,improve neural function,and is safe and effective.
