摘要
李应存教授为国内研究敦煌医学的专家,国家中医药管理局十二五重点建设学科敦煌医学学科带头人,甘肃中医药大学附属医院中医内科专家,在临床中广泛使用敦煌医方,探求敦煌医学辨证思路将理论与实践相结合,造福人民。李应存教授博采众家之长,创立了胃痞病"虚滞"理论,认为现代胃痞病的发病,病位在"胃",当责"肝""脾"二脏,与肺相关,常见病机为脾虚失运,肝气郁滞,中焦气机不利,不利则满,不荣则痛,则见痞满、胀痛的不适症状,"虚滞"理论用于指导西医消化系统疾病的临床治疗,效果突出,其处方选药时特别强调"泻肝实脾",使疏泄与运化的相互为用、藏血与统血的相互协调,以助胃气的恢复。经过长期对胃痞病的治疗和研究,潜心钻研敦煌文献脾胃病的相关论述,结合多年的临床实践,将药物进行重新组合,形成了独特的药物配伍风格,尤其是在敦煌古方中两味药组成的对药和三味药组成的角药运用方面,独具匠心,风格独特。对角药在胃痞病临床治疗上的配伍应用,不仅可以增效减毒,扩大药物的临床使用范围,而且可以执简驭繁,方便辨证选药,使临床治疗有的放矢。通过对李应存教授运用角药部分经验的总结,以探索胃痞病的用药规律,从而扩展治疗思路,提高临床效果。
Professor LI Yingcun is a domestic expert in Dunhuang medicine.Dunhuang prescriptions are widely used in clinical practice.He seeks to combine theory with practice to benefit the people.Professor LI Yingcun has learned from many experts and established the theory of " deficiency and stagnation" of stomach fullness,which is used to guide the clinical treatment of digestive system diseases in Western medicine.The effect is outstanding.When prescribing drugs,he especially emphasizes " purging the liver and strengthening the spleen",making the use of dredging and transporting,coordinating collecting and controllling blood,so as to help the recovery of stomach Qi.In the long-term treatment and research of stomach fullness,combined with many years of clinical practice,the combination of related drugs has formed a unique style of drug compatibility,especially in the use of triangle drugs composed of two or three herbs in Dunhuang ancient prescriptions,which has unique ingenuity and style.The compatible application of triangle drugs in clinical treatment of stomach fullness can not only enhance the efficacy and reduce toxicity,expand the scope of clinical use of drugs,but also simplify and control the complexity,facilitate the selection of drugs based on syndrome differentiation,and make the clinical treatment targeted.This article summarized professor LI Yingcun’s experience in using triangle drugs to explore the rule of medication for stomach fullness so as to expand the thinking of treatment and improve the clinical effect.
