摘要
目的:分析高危型人乳头瘤病毒(HR HPV)感染者的阴道微生态特征,探讨HR HPV感染与阴道微生态失调之间的相关性。方法:收集2015年5月至2017年4月于北京清华长庚医院妇产科同时行阴道微生态及HPV检测的4381例患者的资料,分析微生态检测结果与高危型HPV检测结果之间的相关性。结果:4381例患者中,微生态异常诊断为各种阴道炎者1346例,其中需氧菌性阴道炎(AV)单纯及混合感染839例(839/4381,19.14%),细菌性阴道病(BV)单纯及混合感染508例(508/4381,11.59%),外阴阴道假丝酵母菌病(VVC)单纯及混合感染448例(448/4381,10.23%),滴虫性阴道炎(TV)单纯及混合感染33例(33/4381,0.75%)。HR HPV感染470例,感染前五位为HPV16、58、52、53、39型。AV、BV感染增加HR HPV感染风险(P<0.05);TV、VVC与HR-HPV感染之间无明显相关性。AV、BV与HR HPV16/18型感染之间有一定的相关性(P<0.05)。结论:妇科微生态中AV、BV增加HR HPV感染的风险,尤其是HPV16/18型感染。
Objective:To analyze the characteristics of vaginal microecology in patients with HR HPV infection,to explore the correlation between HR HPV infection and vaginal microecology disorder.Methods:From May 2015 to Apr.2017,patient's data of gynecologic microecology and HPV test were collected from gynecological clinic of Beijing Tsinghua Changgung Hospital.The correlation between microecology and HR HPV was analyzed.Results:A total of 4381 patients were collected,1346 cases of vaginitis were diagnosed.Among them,AV single and mixed infection was 839 cases(839/4381,19.14%),BV 508 cases(508/4381,11.59%),VVC 448 cases(448/4381,10.23%),TV 33 cases(33/4381,0.75%);high-risk HPV infection was 470 cases.AV and BV infection increased the risk of HR HPV infection(P<0.05).TV,VVC and HR HPV infection were not significantly correlated.AV、BV infection and HR HPV 16/18 infection had a certain correlation(P<0.05).Conclusions:AV、BV infection incerase the risk of HR HPV infection,especially HPV16/18.
