摘要
[目的]初步构建机械通气病人使用的ICU获得性衰弱风险评估指标,为早期识别和干预ICU获得性衰弱提供理论依据。[方法]成立科研小组,通过查阅文献,基于奥马哈系统问题分类法,采用专家咨询对ICU获得性衰弱风险评估指标的相关内容进行咨询。[结果]第1轮和第2轮专家咨询有效回收率分别为95.2%(20/21)和100.0%(20/20),权威系数分别为0.855和0.865。专家意见的肯德尔协调系数分别为0.131(P<0.01)和0.209(P<0.01)。最终形成4个维度(环境、社会心理、生理、健康相关行为)43个条目的机械通气病人ICU获得性衰弱风险评估指标。[结论]基于奥马哈系统问题分类法构建的机械通气病人ICU获得性衰弱风险评估指标,能够为早期识别ICU获得性衰弱提供客观指标以及为相关风险评估工具奠定研究基础。
Objective:To initially construct a risk assessment index of ICU-acquired weakness(ICU-AW)among patients with mechanical ventilation,thus,to provide the theoretical basis for early identification and intervention of ICU-AW.Methods:A scientific research group was set up.Then,the problem classification method of Omaha system was taken as the theoretical framework.And relevant literature was reviewed.Finally,related contents of the risk assessment index of ICU-AW were investigated by expert consultation.Results:The effective recovery rates of the first and second rounds of expert consultation were 95.2%(20/21)and 100.0%(20/20),respectively.And the authority coefficients of two rounds of consultations were 0.854 and 0.865,respectively.The Kendall coefficients of expert opinions were 0.131(P<0.01)and 0.209(P<0.01),respectively.Finally,formed ICU-AW risk assessment index among patients with mechanical ventilation consisted of four dimensions,environment,social psychology,physiology,and health-related behaviors,involving 43 items.Conclusions:The ICU-AW risk assessment index among patients with mechanical ventilation based on the problem classification method of the Omaha system,could provide objective indicators for early identification of ICU-AW,and lay a foundation for developing the related risk assessment tools.
