摘要
目的探讨新产程标准对初产妇分娩方式及新生儿不良结局的影响。方法回顾性分析2015年1月至2018年12月我院产科分娩初产妇的病例资料,将2015年1月至2016年12月按照Friedman产程标准管理的234例产妇设置为对照组,2017年1月至2018年12月按照新产程标准管理的213例产妇设置为观察组。比较两组初产妇分娩方式、中转剖宫产原因及新生儿结局。结果观察组中转剖宫产率为15.02%,明显低于对照组的24.79%,差异具有统计学意义(P<0.05)。观察组因产程时限异常中转剖宫产率显著低于对照组,差异有统计学意义(P<0.05)。两组新生儿窒息、新生儿肺炎、转新生儿科率比较,差异无统计学意义(P>0.05)。结论新产程标准管理能够有效减少不必要的产科干预,降低会阴侧切及中转剖宫产率,促进自然分娩,同时不增加新生儿的不良结局。
Objective To explore the influences of new labor standards on delivery mode of primipara and neonatal adverse outcomes.Methods The case data of primipara who delivery in obstetrics department of our hospital from January 2015 to December 2018 were retrospectively analysed.A total of 234 delivery women who were managed with Friedman labor standard from January 2015 to December 2016 were set as control group,213 delivery women who were managed with new labor standards from January 2017 to December 2018 were set as observation group.The delivery mode,the reason of transferring to cesarean section of primipara and neonatal outcomes in the two groups were compared.Results The rate of conversion to cesarean section in the observation group was 15.02%,which was significantly lower than 24.79%in the control group,the difference was statistically significant(P<0.05).The rate of conversion to cesarean section due to abnormal duration of labor in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).There were no significant differences in the rates of neonatal asphyxia,neonatal pneumonia and transfer to neonatal department between the two groups(P>0.05).Conclusion The new standard management of labor process can effectively reduce unnecessary obstetric intervention,reduce the rates of perineal incision and transfer to cesarean section,and promote natural delivery,without increasing the adverse outcome of newborns.
