摘要
目的探讨甲状腺过氧化物酶抗体(TPO-Ab)阳性的亚临床甲状腺功能减退症孕妇孕期治疗的效果评价及临床意义。方法选取2014年1月~2017年1月在我院门诊进行产检筛查出TPO-Ab阳性的亚临床甲状腺功能减退症并在我院住院分娩的110例孕妇,根据治疗方法不同分为实验组与对照组,实验组68例孕妇孕期予左甲状腺素钠片(LT4)50μg,每日1~2次口服,对照组42例孕期未接受甲状腺素补充治疗。对两组孕妇孕期治疗的安全性以及甲状腺功能恢复情况进行临床效果观察和评价。结果实验组孕期促甲状腺激素(TSH)下降明显,与对照组孕妇比较,差异有统计学意义(P<0.05),两组孕妇妊娠期高血压疾病、妊娠期糖尿病发生率比较,差异无统计学意义(P>0.05);实验组分娩时剖宫产发生率与对照组比较差异无统计学意义(P>0.05)。结论TPO-Ab阳性亚临床甲状腺功能减退症孕妇孕期治疗对孕妇甲状腺功能有一定的益处,但对孕妇妊娠期并发症发生率、分娩方式无实际临床意义。
Objective To explore effect evaluation and clinical significance of pregnancy treatment in pregnant women with thyroid peroxidase antibody(TPO-Ab)positive subclinical hypothyroidism.Methods 110 pregnant women who were diagnosed with TPO-Ab positive subclinical hypothyroidism during the prenatal examination at our outpatient clinic and delivered in our hospital from January 2014 to January 2017 were selected.The patients were divided into experimental group and control group according to different treatment methods.68 pregnant women in the experimental group received 50 ug of levothyroxine sodium tablets(LT4)1-2 times a day during pregnancy,while 42 patients in the control group did not receive thyroid hormone supplementation during pregnancy.The clinical effects such as the safety of pregnancy treatment and the recovery of thyroid function in the two groups of pregnant women were observed and evaluated.Results The thyroid stimulating hormone(TSH)of pregnant women in the experimental group decreased significantly during pregnancy,and the difference was statistically significant(P<0.05).There was no difference in the incidence of pregnancy-induced hypertension and gestational diabetes between the two groups(P>0.05).There was no significant difference in the incidence of cesarean section between the experimental group and the control group(P>0.05).Conclusion Pregnancy treatment in pregnant women with TPO-Ab positive subclinical hypothyroidism has certain benefits for thyroid function of pregnant women,but has no practical clinical significance for the incidence of pregnancy complications and delivery methods.
