摘要
目的比较改良的引流条安置方法与传统的引流条安置方法对腋臭小切口顶泌汗腺修剪术术后早期并发症的影响。方法利用抛硬币的方法将入选的81例腋臭患者左右两侧随机分为AB两组。两组均采用小切口顶泌汗腺修剪术。A组采用改良的引流条安置方法,引流条分别从腋窝术区上臂侧、切口、胸壁侧穿出。B组采用传统的引流条安置方法,引流条从上臂侧或胸壁侧穿出。对比腋臭术后早期并发症的发生情况。结果改良的引流条安置方法组在皮下积血评分第一天(1.57±0.76)、第二天(1.23±0.43)、皮瓣贴附(95.06%)、拆线时间(11.33±1.80天)均优于传统组皮下积血评分第一天(1.94±0.99)、第二天(1.49±0.59)、皮瓣贴附(85.19%)、拆线时间(12.41±1.99天),差异有统计学意义(P<0.05)。改良的引流条安置方法组在皮瓣血供评分(1.27±0.48)、感染情况(2.5%)与传统组皮瓣血供评分(1.31±0.54)、感染情况(4.9%)差异无统计学意义(P>0.05)。结论改良的引流条安置方法可以降低腋臭小切口顶泌汗腺修剪术术后皮下积血、皮下空腔形成等早期并发症的发生,有利于腋臭术后的恢复。
Objective To compare the postoperative early complications between improved and conventional methods of drainage for the surgical treatment of axillary bromhidrosis through Mini-Incisions.Methods The two sides of the 81 patients for surgical treatment of axillary bromhidrosis were randomly divided into two groups by using a coin.The patients in Group A were performed with an improved method of drainage which passed through three drainage ports,the upper arm side,the incision side and the chest side of the surgical site.The patients in Group B performed with conventional drainage which passed through one port,the upper arm or chest side.Compare the postoperative early complications between the two groups.Results The hematoma score of the first day(1.57±0.76)and the second day(1.23±0.43),the adhesion of the flap (95.06%) and the time of stitches removal(11.33±1.80d) in Group A were better than the hematoma score of the first day(1.94±0.99)and the second day(1.49±0.59),the adhesion of the flap (85.19%) and the time of stitches removal(12.41±1.99d) in Group B,and the differences were statistical significantly (P﹤0.05).There were no significant differences between the blood supply score(1.27±0.48) and the infection(2.5%) in Group A and the blood supply score(1.31±0.54) and the infection(4.9%) in Group B (P﹥0.05).Conclusion The surgical treatment of axillary bromhidrosis through Mini-Incisions using the improved method of drainage could reduce the occurrence rates of the hematoma,cavity formation under the flap and other postoperative early complications,promote the wound healing and postoperative recovery.
